摘  要:目的比较MATCH序列与磁共振常规序列(3D-TOF,T1WI,T1WI+C)对颈动脉斑块负荷的诊断效能.方法对符合标准的41例患者行3D-TOF,MATCH,T1WI及T1WI增强扫描.采用Vessel Mass软件进行斑块体积(PV)、斑块最大横截面的管腔直径(LD)、管腔面积(LA)、血管总直径(TVD)、血管总面积(TVA)、管壁厚度(WT)、管壁面积(WA)等负荷测量,计算斑块最大横截面的管壁标准化指数(NWI),分别将3D-TOF,T1WI,MATCH序列的斑块负荷指标与增强的T1WI序列进行对比.结果 41例患者在3D-TOF,T1WI,MATCH各序列上测得斑块个数分别为100,95,85,T1WI+C为92; 4种序列检测到的颈动脉斑块左侧分布率差异无统计学意义,且4种序列斑块左侧分布率均>50%; 3D-TOF的PV,LA与T1WI+C序列差异具有明显的统计学意义(P<0.05); T1WI,MATCH序列的PV,LD,LA,TVD,TVA,WT,WA,NWI与T1WI+C比较差异均无统计学意义(P>0.05).结论 3DTOF序列对斑块的检测存在假阳性,且对于斑块负荷检测有限.颈总动脉解剖学的差异造成了颈动脉左侧斑块多发,MATCH序列颈动脉斑块的负荷检测与增强T1WI序列结果具有较好的一致性.Objective To compare the diagnostic efficiency of MATCH sequence and conventional MRI sequence(3DTOF,T1WI,T1WI+C)for carotid plaque burden.Method According to inclusion criteria,41 patients with plaque confirmed by carotid artery color Doppler ultrasound were selected.MATCH sequence,conventional sequence and enhanced scan were performed.Post processing with VesselMass software.Plaque load was measured in each sequence,including plaque volume(PV),lumen diameter(LD),lumen area(LA),total vessel diameter(TVD),total vessel area(TVA),wall thickness(WT)and wall area(WA).The normalized wall index(NWI)of the maximum cross section of the plaque was calculated according to the formula.The plaque load indices of 3D-TOF sequences,T1WI sequences and MATCH sequences were compared with enhanced T1WI sequences.Results In 41 patients,100 plaques were detected by 3D-TOF sequence,95 plaques by T1WI sequence,85 plaques by MATCH sequence,and 92 plaques by T1WI+C sequence.There was no significant difference in the left distribution rate of carotid plaque detected by four sequences,and the left distribution rate of carotid plaque detected by four sequences was more than 50%.There were significant differences in PV and LA of 3D-TOF sequence and T1WI+C sequence(P<0.05).There was no significant difference in PV,LD,LA,TVD,TVA,WT,WA,NWI between T1WI sequence and T1WI+C sequence(P>0.05).Conclusion 3D-TOF sequence has false positive results for plaque detection and limited detection of plaque load.The difference in the anatomy of the common carotid artery resulted in frequent plaque on the left side of the carotid artery.MATCH sequence and the enhanced T1WI have better consistency in the load detection of carotid artery plaques.
