自拟湿热阻络方治疗糖尿病下肢周围神经病变的临床分析

摘　　要：目的探讨自拟湿热阻络方治疗糖尿病下肢周围神经病变的临床效果。方法选取我院2016年1月～2018年1月接收的65例糖尿病下肢周围神经病变患者作为此次研究对象,按照随机数字表法将其分为对照组(n=32)与治疗组(n=33),两组患者均接受规范性西医治疗,对照组采用西药依帕司他治疗,治疗组采用自拟湿热组络方剂浴足治疗,比较两组临床效果。结果治疗组总有效率明显高于对照组,差异有统计学意义(P <0.05);治疗前,两组间各项指标比较差异无统计学(P> 0.05),治疗后,两组证候积分与体征积分较治疗前均降低,且治疗组明显低于对照组,差异有统计学意义(P <0.05);治疗前两组SCV与NCV比较差异无统计学意义(P> 0.05),治疗后治疗组SCV(正中神经、腓总神经、胫神经)以及NCV(正中神经、腓总神经)传导速度均高于对照组,差异有统计学意义(P <0.05)。结论自拟湿热阻络方能有效改善糖尿病下肢周围神经病变的临床症状,提高神经传导速度,值得在临床推广实施。Objective To explore the clinical effect of self-made damp-heat blocking collaterals prescription in treatment of diabetic peripheral neuropathy of lower limbs. Methods Sixty-five patients with diabetic peripheral neuropathy of lower limbs who were admitted to our hospital from January 2016 to January 2018 were selected as research objects.According to the random number table method,they were randomly divided into the control group(n=32)and the treatment group(n=33).Patients in both groups received normative treatment of western medicine. The control group was treated with epalrestat while the treatment group was treated with self-made damp-heat blocking collaterals prescription for feet bath.The clinical curative effects were compared between two groups. Results The total effective rate in the treatment group was higher than that in the control group,and the difference was statistically significant(P < 0.05).Before treatment,there was no statistical difference in each index between the two groups(P > 0.05).After treatment,the scores of syndromes and signs in the two groups were lower than those before treatment,and the scores in the treatment group were significantly lower than those in the control group.The difference were statistically significant(P < 0.05).There was no statistically significant difference between SCV and NCV before treatment(P > 0.05).After treatment,the conduction velocity of SCV(median nerve,common peroneal nerve,sacral nerve)and NCV(median nerve,common peroneal nerve)in the treatment group were higher than those in the control group.The difference was statistically significant(P < 0.05). Conclusion Self-made damp-heat blocking collaterals prescription can efficiently improve the clinical symptoms of diabetic peripheral neuropathy of lower limbs and improve nerve conduction velocity,which is worthy of clinical promotion and application.
