阴道超声引导下子宫输卵管声学造影在输卵管阻塞中的临床研究

摘　　要：目的:探讨阴道超声引导下子宫输卵管声学造影在输卵管阻塞中的临床应用价值。方法:收集妇科门诊及住院的输卵管阻塞患者各65例,分别记为对照组和观察组。其中对照组行传统输卵管通液术诊疗处理,观察组采用经阴道超声引导下子宫输卵管声学造影术处理。以腹腔镜结果为金标准,比较两组诊断输卵管阻塞的准确率;术后均随访6个月,统计两组妊娠情况。结果:观察组能实时动态观察输卵管腔情况,诊断输卵管通畅程度准确率100.00%,明显高于对照组的52.31%,差异有统计学意义(χ~2=40.707,P<0.001);术后均成功随访6个月,观察组妊娠率达55.38%,明显高于对照组36.92%,差异有统计学意义(χ~2=4.457,P<0.05)。结论:阴道超声引导下子宫输卵管声学造影术能准确诊断输卵管阻塞情况和提高术后妊娠率,值得临床推广应用。Objective:To investigate the clinical value of transvaginal ultrasound-guided hysterosalpingography in fallopian tube obstruction. Methods:65 cases of fallopian tube obstruction in outpatient and 65 cases of inpatient department of gynecology were collected and recorded as control group and observation group,respectively.The control group was treated with traditional tubal drainage,while the observation group was treated with transvaginal ultrasound-guided hysterosalpingography.Laparoscopic results were used as the gold standard,the accuracy of diagnosis of fallopian tube obstruction was compared between the two groups.All patients were followed up for 6 months after operation,and the pregnancy status of the two groups was counted. Results:The observation group was able to observe the condition of fallopian tube in real time and dynamically,and the accuracy rate of diagnosis of fallopian tube patency was 100.00%, which was significantly higher than 52.31% of the control group (χ^2 =40.707,P <0.001).After 6 months of follow-up,the pregnancy rate of the observation group was 55.38%,which was significantly higher than that of the control group (36.92%),and the difference was statistically significant (χ^2=4.457,P <0.05). Conclusion:Vaginal ultrasound-guided hysterosalpingography can accurately diagnose fallopian tube obstruction and improve the pregnancy rate after operation.Its significant value is worthy of clinical application.
