增强型体外反搏对缺血性心力衰竭患者心肾功能及内分泌系统的影响

摘　　要：目的:探讨增强型体外反搏(enhanced external counterpulsation,EECP)对缺血性心力衰竭患者心肾功能及内分泌系统的影响。方法:根据随机数表法将106例缺血性心力衰竭患者分为两组,对照组(n=53)采用药物治疗,观察组(n=53)在对照组治疗方法基础上采用EECP治疗。对比两组治疗前后心功能、肾功能、神经内分泌因子的差异。结果:治疗后,两组患者心率、LVEDV、LVESV、Tei显著下降,6MMWD、LVEF显著上升;且观察组优于对照组,差异有统计学意义(均P<0.05)。治疗后,两组患者Cys-C、sCr、NT-proBNP显著下降,eGFR显著上升;且观察组均优于对照组,差异有统计学意义(均P<0.05)。治疗后,两组患者AngⅡ、ALD、PRA显著下降;且观察组低于对照组,差异有统计学意义(均P<0.05)。观察组与对照组不良反应发生率分别为13.21%和11.32%,差异无统计学意义(P>0.05)。结论:EECP可明显改善缺血性心力衰竭患者心肾功能和神经内分泌功能。Objective:To investigate the effects of enhanced external counterpulsation (EECP) on cardiorenal function and endocrine system in patients with ischemic heart failure. Methods:106 patients with ischemic heart failure divided into two groups according to the random number table method.The patients in the control group (n =53) were treated with drugs,while the patients in the observation group (n =53) were treated with EECP on the basis of the control group.The differences of cardiac function,renal function and neuroendocrine factors between the two groups before and after treatment were compared. Results:After treatment,the heart rate,LVEDV,LVESV and Tei of the two groups decreased significantly,while 6MMWD and LVEF increased significantly,and the observation group was superior to the control group,the difference was statistically significant ( P <0.05).After treatment,Cys-C,sCr and NT-proBNP decreased significantly and eGFR increased significantly in the two groups,and the observation group was better than the control group,the difference was statistically significant (P <0.05).After treatment,Ang II,ALD and PRA of the two groups decreased significantly,and the observation group was lower than the control group,the difference was statistically significant ( P <0.05).The incidence of adverse reactions in the observation group and the control group were 13.21% and 11.32%,respectively,with no significant difference ( P >0.05). Conclusion:EECP can significantly improve cardiorenal function and neuroendocrine function in patients with ischemic heart failure.
