摘 要:①目的探讨多层螺旋CT血管造影术(MSCTA)对颅内动脉瘤的诊断价值,并与数字减影血管造影术(DSA)对比分析。②方法回顾性分析42例经DSA确诊的颅内动脉瘤患者的MSCTA影像资料,研究动脉瘤的位置、形态以及动脉瘤瘤颈开口与载瘤动脉的连接方式及其分支血管的空间结构关系。③结果42例颅内动脉瘤患者,其中单发35例,多发7例,共发现56个动脉瘤。按部位分为颈内动脉瘤27个(27/56,48.21%),大脑前动脉瘤14个(14/56,25.00%),大脑中动脉瘤13个(13/56,23.21%),大脑后动脉瘤1例,后交通动脉瘤1例;按形态分为囊型动脉瘤39个(39/56,69.42%),梭型动脉瘤4个(4/56,0.93%),其他类型动脉瘤13个(13/56,23.21%);MSCTA重建成像诊断颈内动脉瘤的敏感性、特异性、准确度分别为98.14%、92.86%、97.54%。④结论MSCTA能够显示颅内动脉瘤数目、形态、结构及其分支开口的空间结构关系,并能够显示动脉瘤周围神经解剖学结构,为颅内动脉瘤的早期诊断及制定治疗方案提供了可靠依据,具有较高的诊断价值。Objective To explore the diagnosis values of MSCTA for intracranial aneurysms,and compare with DSA. Methods The MSCTA imaging data of 42 patients with intracranial aneurysm diagnosed by DSA were analyzed retrospectively,the location and morphology of the aneurysm and its relationship with the spatial structure of the artery carrying the tumor were studied. Results A total of 56 aneurysms were found in 42 patients,35 cases were single and 7 cases were multiple.According to the site where the aneurysm occurred,it was divided into 27 intracranial aneurysms(27/56,48.21%),14 anterior cerebral aneurysms(14/56,25.00%),13 cerebral aneurysms(13/56,23.21%),1 case was located in the posterior cerebral aneurysm,1 case of posterior traffic aneurysm.39 blister aneurysms(39/56,69.42%),4 fusiform aneurysm(4/56, 0.93%),13 irregular aneurysm(13/56,23.21%).The sensitivity,specificity,and accuracy of MSCTA in diagnosing intracranial aneurysms were 98.14%, 92.86% and 97.54%. Conclusion MSCTA can show the number,morphology,structure and spatial structure of the aneurysm.It can also show the anatomical structure of peripheral nerve and provide a reliable basis for early diagnosis and treatment of intracranial aneurysms,and has a high diagnostic value.
