摘 要:目的探讨老年2型糖尿病(T2DM)患者糖化白蛋白(GA)、GA/糖化血红蛋白(HbA1c)、血清胱抑素(Cys)C水平与颈动脉粥样硬化的关系。方法老年T2DM患者428例,依据有无颈动脉粥样硬化分为单纯T2DM组219例,T2DM合并颈动脉粥样硬化组209例。比较一般资料、生化指标、C肽水平、GA、HbA1c、GA/HbA1c比值及CysC水平,分析各指标与T2DM颈动脉粥样硬化病变之间的关系。结果糖尿病病程、收缩压(SBP)、体重指数(BMI)、低密度脂蛋白胆固醇(LDL-C)、空腹血糖(FPG)、餐后2 h血糖(2 h PG)、GA、GA/HbA1c、CysC水平在T2DM合并颈动脉粥样硬化组显著高于单纯T2DM组(P<0.05)。GA和2 h PG、HbA1c之间存在正相关,与BMI及空腹C肽呈负相关,相关系数分别为r=0.667、0.658、-0.381、-0.321,均P<0.05。GA/HbA1c与GA、年龄、糖尿病病程呈正相关,与HbA1c呈负相关,相关系数分别为(r=0.546、0.196、0.228、-0.180,均P<0.05)。Logistic回归分析结果:GA、GA/HbA1c、CysC与T2DM颈动脉粥样硬化独立相关。结论 GA、GA/HbA1c、CysC水平变化与T2DM颈动脉粥样硬化的发生密切相关。Objective To investigate the relationship between glycated albumin (GA), GA/HbA1c levels, CysC and carotid atherosclerotic macrovascular complications in elderly patients with type 2 diabetes mellitus. Methods Subjects were selected from hospitalized elderly patients with type 2 diabetes, a total of 428 patients, divided into two groups according to the presence or absence of carotid atherosclerosis. There were 219 patients in the T2DM group and 209 patients in the T2DM group with carotid atherosclerosis. More general information, biochemical indicators, C peptide levels, GA, HbA1c GA/ HbA1c and CysC levels were compared, and the relationship between various indicators and carotid atherosclerotic lesions were analyzed. Results Diabetic course, SBP, BMI, LDL-C, FPG, 2 h PG, GA, GA/HbA1c and CysC were found to be higher in T2DM combined with carotid atherosclerosis group than those of non-carotid atherosclerosis group( P <0.05). There was positive correlation between GA and 2 h PG and HbA1c, and negative correlation with BMI and fasting C peptide, and the correlation coefficients were respectively 0.667, 0.658,-0.381 and -0.321. GA/HbA1c was positively correlated with GA, age and diabetes course, negative correlation with HbA1c, the correlation coefficients were respectively 0.546, 0.196, 0.228 and -0.180. Logistic regression analysis results showed that GA, GA/HbA1c, CysC were independently correlated with carotid atherosclerosis in type 2 diabetes. Conclusions Changes in GA, GA/HbA1c and CysC levels are closely related to the occurrence of carotid atherosclerosis in type 2 diabetes mellitus.
