糖化血红蛋白、果糖胺与胰岛细胞抗体联合检测对老年糖尿病肾病的诊断价值

摘　　要：目的分析糖化血红蛋白(HbA1c)、果糖胺(FMN)与胰岛细胞抗体(ICA)联合检测对老年糖尿病肾病的诊断价值。方法 100例糖尿病肾病患者为观察组,100例健康体检者为对照组;检测血清中HbA1c、FMN与ICA水平,分析各指标单独及联合诊断的价值。结果观察组血清中HbA1c、FMN与ICA水平均明显高于对照组(P<0.05);平行联合(任一指标阳性)诊断的敏感度显著高于各指标单独诊断(P<0.05),系列联合(所有指标均阳性)诊断的特异度显著高于各指标单独诊断,平行联合及系列联合诊断的曲线下面积显著高于各指标单独应用。结论对老年糖尿病肾病患者采用HbA1c、FMN与ICA联合诊断可有效提高诊断敏感性及特异度。Objective To analyze the diagnostic value of combined detection of HbA1c, FMN and ICA in elderly diabetic nephropathy patients. Methods 100 patients with diabetic nephropathy were selected as the observation group and 100 healthy persons as the control group. The levels of serum HbA1c, FMN and ICA were measured, and the diagnostic value of the individual diagnosis and joint diagnosis of each index was analyzed. Results The levels of HbA1c, FMN and ICA of observation group were significantly higher than those in control group( P <0.05);there were 101 positive cases in HbA1c diagnosis, 102 cases positive in the diagnosis of FMN, 96 positive in ICA diagnosis, 120 cases with parallel combined diagnosis and 83 cases with combined diagnosis, and the sensitivity of parallel combined diagnosis was obvious. The specificity of the series combined diagnosis was significantly higher than that of the individual diagnosis ( P <0.05), and the area under the curve of parallel and series combined diagnosis was significantly higher than that of the individual diagnosis ( P <0.05). Conclusions The combination of HbA1c, FMN and ICA could effectively improve the diagnostic sensitivity and specificity of the elderly diabetic nephropathy patients, and has high clinical value.
