输卵管妊娠手术治疗后不同生殖状况的相关因素研究

摘　　要：目的研究输卵管妊娠手术治疗后对患者生殖状况的影响因素。方法选取2014年8月至2016年8月解放军第163中心医院诊治的184例输卵管妊娠手术患者作为研究对象。根据患者术后生育情况进行分析。采集患者一般资料,对比妊娠成功和妊娠失败患者一般资料,将有差异项目带入Logistic回归方程计算,分析输卵管妊娠手术后影响生殖状况的因素。结果 184例患者术后随访1年,其中妊娠成功患者139例,占比75.5%;妊娠失败患者45例,占24.5%。妊娠失败患者中,流产患者11例(6.0%),异位妊娠患者13例(7.1%),未妊娠患者21例(11.4%)。妊娠成功患者与妊娠失败患者在年龄、文化程度、盆腔炎病史、异位妊娠史、输卵管粘连和输卵管伞端闭锁比较,其差异均具有统计学意义(均P<0.05)。将上述有差异项目带入Logistic回归方程计算,发现年龄、文化程度、盆腔炎病史、异位妊娠史、输卵管粘连和输卵管伞端闭锁,均是输卵管妊娠手术治疗患者不同生殖状况的影响因素。结论输卵管妊娠手术治疗患者术后生殖状况的影响因素较多,给予针对性措施进行干预,可以有效提高术后患者的妊娠成功率。ObjectiveTo study the influence factors of reproduction conditions after the operation of tubal gestation. Methods184 patients received the operation of tubal gestation from August 2014 to August 2016 in People′s Liberation Army 163 Central Hospital were selected. The postoperative reproduction conditions were observed. According to the Logistic regression equation analysis of successful and failed cases, the influence factors of reproduction conditions were analyzed. ResultsAfter a year′s follow-up visit, the rate of successful pregnancy was 75.5%(139 cases) while the rate of failed pregnancy was 24.5%(45 cases). For failed cases, there were 11 abortion cases (6.0%), 13 cases of ectopic gestation (7.1%) and 21 non-pregnancy cases (11.4%). The age, education background, history of pelvic inflammation, ectopic gestation, fallopian tube adhesion and fallopian fimbriated extremity interlocking of successful cases and failed cases were significantly, with statistically significant differences (all P<0.05). According to the Logistic regression equation analysis, the influence factors of reproduction conditions included age, education background, history of pelvic inflammation, ectopic gestation, fallopian tube adhesion and fallopian fimbriated extremity interlocking. ConclusionsAfter the operation of tubal gestation, there are multiple influence factors of reproduction conditions and specified nursing interventions can improve the successful pregnancy rate.
