摘 要:目的了解中山市2017年沙门菌的耐药特征及分子分型,为临床合理用药和食源性疾病的溯源提供科学依据。方法对中山市2017年食源性疾病监测(3个哨点医院)的沙门菌分离株采用微量肉汤稀释法进行14种抗生素药敏试验,运用脉冲场凝胶电泳(PFGE)技术对肠炎沙门菌进行分子分型。结果中山市2017年食源性疾病监测检出的285株沙门菌对14种抗生素敏感性试验结果显示,237株(占83.16%)沙门菌对13种抗生素呈现不同程度的耐药性,未发现对亚胺培南(IPM)耐药菌株。耐药抗生素最多的种类达到11种,耐药率较高(40%以上)的抗生素为氨苄西林(AMP)、四环素(TET)、氨苄西林/舒巴坦(SAM)、氯霉素(CHL);共有87类耐药谱型,其中多重耐药谱型75类(86.21%),多重耐药菌株数为176株(61.75%)。25株肠炎沙门菌的PFGE分为5种带型,相似度为63.9%~100.0%。结论中山市2017年食源性疾病监测显示沙门菌耐药状况较为严重,呈多重性和多态性,应采取综合措施控制耐药性;肠炎沙门菌PFGE的相似度较高,存在优势带型。Objective To characterize the antimicrobial resistance and molecular type of Salmonella isolates in Zhongshan City in 2017,and provide information for treatment and traceability of food borne diseases.Methods Broth microdilution method were used for 285 strains of Salmonella isolates to detect the susceptibility to 14 antibiotics in foodborne disease surveillance(3 sentinel hospitals)in Zhongshan,2017,and PFGE was used to confirm molecular type of Salmonella enteritidis.Results Totally 237 strains(83.16%)of isolates showed different antimicrobial resistant levels to 13 antimicrobials tested,among which resistance to as much as 11 classes of antimicrobials was unfolded.The resistance rates to ampicillin(AMP),tetracycline(TET),ampicillin-sulbactam(SAM),chloramphenicol(CHL)were as high as above 40%,while all strains were susceptible to imipenem(IPM).There were 87 antibiograms,176(61.75%)strains were multidrug resistant with 75(86.21%)antibiograms.A cluster analysis was performed on 25 strains of Salmonella enteritis and resulted in 5 PFGE types with similarity range between 63.9%-100.0%.Conclusion Food-borne disease surveillance in Zhongshan in 2017 showed that the multidrug resistance of Salmonella in Zhongshan was rather high with diversity.So that relevant department should control the development of antimicrobial resistance.The PFGE of Salmonella enteritidis showed relatively high similarity with dominant type.
