神经母细胞瘤患儿ABO血型抗体减弱及原因分析

摘　　要：目的分析神经母细胞瘤患儿ABO血型抗体减弱情况及其外周血B细胞和CD4^+T细胞的表达,探讨神经母细胞瘤患儿血型抗体减弱的原因。方法对2015年11月—2017年11月,送往本科室的478例1岁以上临床确诊为神经母细胞瘤的患儿EDTA抗凝全血标本进行ABO血型鉴定,流式细胞术检测B细胞及CD4^+T细胞的比例。结果 478例标本中,97例反定型抗体减弱(A型52例,B型30例,O型15例),且年龄越小抗体减弱程度越高(z=15.46,P<0.01)。同时,神经母细胞瘤患儿外周血B细胞及CD4^+T细胞百分比明显低于对照组(t=26.36,P<0.01)。结论神经母细胞瘤患儿ABO血型抗体减弱与年龄及T、B淋巴细胞比例降低具有一定的相关性。Objective To make a systemic study between the reduced level of ABO blood group antibodies and the expression of B cells and CD4^+ T lymphocyte in children with neuroblastoma, and to study the possible causes. Methods From November 2015 to November 2017, pre-transfusion test of ABO blood grouping were performed on 478 children, all over 1 year old, diagnosed clinically as neuroblastoma. And the expression of B cells and CD4^+ T lymphocyte were evaluated by flow cytometry. Results Of the 478 samples, decreased antibody levels during ABO reverse typing were found in 97 cases(52 cases in type A, 30 in type B and 15 in type O). The younger the age, the higher decrease in antibody level(z=15.46, P<0.01). In addition, the percentage of B cells and CD4^+ T cells in peripheral blood of children with neuroblastoma was significantly lower than that of the control group(t=26.36, P<0.01). Conclusion The decrease of ABO blood group antibody level in children with neuroblastoma is correlated with age and the decreased proportion of T and B lymphocytes.
