参苓白术散加减辅助治疗难治性肾病综合征脾肾阳虚证疗效及对血清SAA和IL-6水平影响

摘　　要：目的:探讨参苓白术散加减辅助治疗难治性肾病综合征(RNS)脾肾阳虚证的疗效及对血清淀粉样蛋白A(SAA)和白细胞介素(IL)-6水平的影响。方法:将本院就诊的RNS患者58例按数字表法随机分为对照组(29例)和治疗组(29例)。两组给予常规干预措施和强的松。对照组采取环磷酰胺静滴,10 mg·kg^(-1)·d^(-1),每周2次。治疗组在对照组基础上采取参苓白术散加减治疗,1剂/d,2次/d。两组患者连续治疗12周。比较两组脾肾阳虚证症状评分和临床疗效。采取生化分析仪检测两组24 h尿蛋白定量、血浆白蛋白、总胆固醇、甘油三酯水平。检测两组血清SAA和IL-6水平。结果:治疗后,治疗组脾肾阳虚证症状评分显著低于对照组(P<0.01)。治疗组治疗后24 h尿蛋白定量、甘油三酯、总胆固醇及症状评分明显少于对照组,血浆白蛋白显著高于对照组(P<0.01)。治疗组总有效率为93.10%,明显优于对照组为65.52%(P<0.05)。治疗组治疗后血清SAA和IL-6水平显著少于对照组(P<0.01)。结论:参苓白术散加减辅助治疗RNS脾肾阳虚证可促进症状体征改善,提高临床疗效,调节患者体内SAA和IL-6水平可能与其疗效有关。Objective:To investigate the efficacy of Shenling Baizhu Powder in treatment of refractory nephritic syndrome(RNS)with syndrome of spleen and kidney Yang deficiency and its influence on serum levels of amyloid A(SAA)and interleukin(IL)-6. Methods:Fifty-eight RNS cases with syndrome of Yang deficiency of spleen and kidney were randomly divided into control group(29 cases)and treatment group(29 cases). Both groups received rontine combined with prednisone. Cases of control group were treated with intravenous drip of cyclophosphamide(10 mg·kg-1·d-1 and 2 times/week). On basis of control group,treatment group was treated with Shenling Baizhu Powder(1 dose/d and bid). Both groups were observed for 12 weeks. Symptoms score of syndrome of spleen and kidney Yang deficiency and the efficacy were compared for both groups. 24 hours urinary protein,plasma-albumin,total cholesterol,and triglyceride were detected by biochemical analyzer. Serum levels of SAA and IL-6 were detected for two groups. Results:After treatment,symptoms score of syndrome of spleen and kidney yang deficiency were evidently lower than control group(P<0.01). 24 hours urinary protein,total cholesterol,and triglyceride of treatment group were obviously lower,while plasma-albumin was higher than control group(P<0.01).Total effective rate of treatment group was 93.10%,which was superior to that of control group 65.52%(P<0.05). After treatment,serum levels of SAA and IL-6 of treatment group were evidently lower than those of control group(P<0.01). Conclusion:Shenling Baizhu Powder in treatment of RNS with syndrome of spleen and kidney Yang deficiency can promote improvement of symptoms and signs,increase the efficacy,and regulation of levels of SAA and IL-6 may be related with the efficacy.
