复荣通脉胶囊辅助治疗对糖尿病下肢血管病变患者下肢血流动力学状态、动脉内膜影响观察

摘　　要：目的:探讨复荣通脉胶囊辅助治疗对糖尿病下肢血管病变患者下肢血流动力学状态、动脉内膜的影响。方法:选取本院2015年10月—2017年9月期间内分泌科收治的糖尿病下肢血管病变患者102例为研究对象,根据随机数字表法随机分为观察组(n=51)和对照组(n=51)。对照组患者给予西洛他唑片治疗,观察组在对照组基础上给予复荣通脉胶囊治疗,两组均连续治疗12周。比较两组治疗疗效,治疗前后中医症状评分、步行受损问卷(Waling Impairment Questionnare,WIQ)量表评分、下肢血流动力学指标变化、动脉内膜状态评分。结果:观察组治疗总有效为90.19%,高于对照组,差异有统计学意义(P<0.05);治疗后,观察组中医症状评分为(6.28±2.49)分,低于对照组,WIQ评分为(0.82±0.13)分,高于对照组,差异均有统计学意义(P<0.05);治疗后,观察组血管内径和血流量分别为(1.45±0.32)mm和(47.74±15.52)mL/min,大于对照组,差异有统计学意义(P<0.05);治疗后,观察组动脉内膜厚度、硬化强度、动脉狭窄程度、斑块评分分别为(0.91±0.09)分、(0.68±0.36)分、(1.26±0.32)分和(1.02±0.16)分,低于对照组,差异均有统计学意义(P<0.05)。结论:糖尿病下肢血管病变患者采用复荣通脉胶囊辅助治疗可有效改善患者下肢血流动力学状态,修复动脉内膜,从而提高治疗效果。Objective:To investigate the effects of Furong Tongmai Capsule adjuvant therapy on hemodynamics status and endarterium of lower extremity in patients with diabetic lower extremity vascular disease. Methods:102 cases of patients with diabetic lower extremity vascular disease treated in endocrinology department of our hospital from October 2015 to September 2017 were selected for the study and were divided into the observation group(n=51)and the control group(n=51)according to random number table method. The control group was treated with cilostazol tablet,and the observation group was given Furong Tongmai Capsule on the basis of the control group,and the two groups were treated continuously for 12 weeks. The treatment efficacy,the TCM symptoms score before and after treatment,the waling impairment questionnare(WQI)score,the hemodynamics indexes of lower extremity and the endarterium status score were compared between the two groups. Results:The total effective rate of treatment was 90.19% in the observation group,which was higher than that in the control group(P<0.05).After treatment,the TCM symptoms score with(6.28±2.49)points in the observation group was lower than that in the control group,while the WIQ score with(0.82±0.13)points was higher than that in the control group(P<0.05). After treatment,the vascular diameter and blood flow volume were(1.45±0.32)mm and(47.74±15.52)mL/min respectively in the observation group,which were significantly higher than those in the control group(P<0.05). After treatment,the scores of endarterium thickness,sclerosis intensity,arterial stenosis degree and plaque were(0.91±0.09)points,(0.68±0.36)points,(1.26±0.32)points and(1.02±0.16)points respectively in the observation group,which were lower than those in the control group(P<0.05). Conclusion:Furong Tongmai Capsule adjuvant therapy for patients with diabetic lower extremity vascular disease can effectively improve hemodynamics status of lower extremity,and repair the endarterium,thereby enhancing the treatment effects.
