补脾凉血活血方对紫癜性肾炎患儿血液流变学及淋巴细胞免疫的影响

摘　　要：目的:探讨补脾凉血活血方对紫癜性肾炎患儿血液流变学及淋巴细胞免疫的影响。方法:2016年3月—2017年11月选择本院紫癜性肾炎患儿88例,电脑随机分为对照组和观察组,分别44例,对照组采用抗血小板、抗过敏等常规治疗;观察组在此基础上采用补脾凉血活血方治疗,持续治疗3个月。比较两组间临床疗效、外周血CD_3^+、CD_4^+、CD_8^+、CD_4^+/CD_8^+及血浆黏滞度、血小板最大聚集率、纤维蛋白原等,记录不良反应状况。结果:对照组有效率为77.27%,观察组有效率为93.18%,两组间有效率比较,差异有统计学意义(P<0.05)。治疗后对照组和观察组外周血CD_3^+分别为(57.24±7.03)%、(62.38±6.89)%,外周血CD_4^+分别为(38.61±3.80)%、(41.93±3.94)%,外周血CD_8^+分别为(24.68±2.03)%、(22.37±1.97)%,外周血CD_4^+/CD_8^+分别为(1.45±0.27)、(1.73±0.29),血浆黏滞度分别为(1.53±0.41)m Pa·s、(1.22±0.35)m Pa·s,血小板最大聚集率分别为(53.69±8.14)%、(46.58±8.05)%,纤维蛋白原分别为(3.10±0.37)g/L、(2.89±0.34)g/L,对照组和观察组上述指标比较,差异有统计学意义(P<0.05)。对照组不良反应率13.64%(6/44)与观察组不良反应率18.18%(8/44)比较,差异无统计学意义(P>0.05)。结论:补脾凉血活血方对紫癜性肾炎患儿疗效显著,能有效改善血液流变学,提高淋巴细胞免疫功能,且患儿可以耐受。Objective: To explore the effect of Bupi Liangxue Huoxue Decoction in the treatment of children with Henoch Schonlein purpura nephritis on hemorheology and lymphocyte immunity. Methods: Eighty-eight children with Henoch Schonlein nephritis from March 2016 to November 2017 were selected,and randomly divided into two groups,44 cases in each. Control group received antiplatelet and anti allergy routine treatment,and the study group additionally received Bupi Liangxue Huoxue Decoction,and two groups were treated for 3 months. Clinical effect,levels of CD3+,CD4+,CD8+,CD4+/CD8+and plasma viscosity,the maximum platelet aggregation rate,fibrinogen and adverse reactions were compared between two groups. Results: The effective rate of the control group( 77. 27%) was lower than the study group’s( 93. 18%) and the difference was statistically significant( P< 0. 05). After treatment,levels of the peripheral blood CD3+level in the control group and the study group was( 57. 24 ± 7.03)% and( 62. 38 ± 6. 89)%,respectively and CD4+levels were( 38. 61 ± 3. 80)% and( 41. 93 ± 3. 94)%,respectively and CD8+levels were( 24. 68 ± 2. 03)% and( 22. 37 ± 1. 97)%,respectively and CD4+/CD8+were 1. 45 ± 0. 27 and 1. 73 ± 0. 29 and the degrees of platelet aggregation were( 1. 53 ± 0. 41) m Pa·s and( 1. 22 ± 0. 35) m Pa·s. The maximum platelet aggregation rates were( 53. 69 ± 8. 14)% and( 46. 58 ± 8. 05)%,respectively and fibrinogen levels were( 3. 10 ± 0. 37) g/L and( 2. 89± 0. 34) g/L,respectively. All the indexes between two groups had differences( P < 0. 05). The adverse reaction rate in the control group was 13. 64%( 6/44) and the study group’s was 18. 18%( 8/44)( P > 0. 05). Conclusion: Bupi Liangxue Huoxue Decoction in the treatment of children with Henoch Schonlein purpura nephritis was effective,and it can effectively improve blood rheology and improve the immune function of lymphocyte,and also can be tolerated in children.
