POTTS评分在临床围手术患者输血中的应用

摘　　要：目的:运用围术期输血指征评分标准(POTTS)指导临床安全合理用血。方法:300例围手术期患者随机分为观察组与对照组,每组150例。观察组运用POTTS指导输血,对照组运用《临床输血技术规范(2000年)》指导输血,对比两组患者围手术期输血量及输血不良反应的发生率。结果:观察组患者悬浮少白细胞红细胞、病毒灭活血浆、冷沉淀因子及单采血小板的输注量均显著低于对照组(P <0.01);观察组代浆液的使用高于对照组(P <0.01);观察组有一例患者出现输血反应,对照组无输血不良反应,两组输血反应比较无统计学差异(P> 0.05)。结论:POTTS用于指导围手术期患者输血安全有效,避免了不必要的输血,减少了血资源的浪费,值得推广。Objective:To explore the application of peri-operative blood transfusion trigger score(POTTS)to guide blood transfusion safely and rationally.Methods:A total of 300 patients in perioperative period were randomly divided into observation group and control group,with 150 cases in each group.The observation group received blood transfusion guided by POTTS and the control group was guided by Clinical Blood Transfusion Technical Standards(2000).The peri-operative blood transfusion volume and the incidence of adverse reactions were compared between the two groups.Results:The erythrocyte,virus inactivated plasma,cryoprecipitation factor and platelet transfusion volume in the observation group were significantly lower than those in the control group(P<0.01).The use of the plasma replacement in the observation group was higher than that in the control group(P<0.01).One patient in the observation group showed transfusion reaction,and there was no significant difference between the two groups in transfusion reaction(P>0.05).Conclusions:POTTS is safe and effective in guiding peri-operative blood transfusion,avoiding unnecessary blood transfusion,reducing the waste of blood resources.
