加味六味地黄汤联合尿激酶治疗早中期慢性肾功能衰竭的临床研究

摘　　要：目的:观察加味六味地黄汤联合尿激酶治疗早中期慢性肾功能衰竭的临床疗效。方法:将114例早中期慢性肾功能衰竭患者随机分为两组,对照组57例采用尿激酶治疗,治疗组57例采用加味六味地黄汤联合尿激酶治疗,两组均连续治疗4周。比较两组治疗后临床疗效,检测两组治疗前后肾功能、凝血功能和微炎症状态指标水平变化情况。结果:治疗后,治疗组临床治疗总有效率显著高于对照组(P<0.05);两组血肌酐(SCr)和尿素氮(BUN)、纤维蛋白原(Fib)以及血清肿瘤坏死因子α(TNF-α)、白细胞介素6(IL-6)和C-反应蛋白(CRP)水平均较治疗前显著降低(P<0.05),且治疗组上述指标均显著低于对照组(P<0.05)。两组内生肌酐清除率(CCr)、凝血活酶时间(APTT)均较治疗前显著升高(P<0.05),且治疗组上述指标均显著高于对照组(P<0.05)。结论:加味六味地黄汤联合尿激酶治疗早中期慢性肾功能衰竭,疗效确切,同时能明显增强肾功能和凝血功能,改善微炎症状态。Objective: To observe the clinical effect of Modified Liuwei Dihuang Decoction combined with Urokinase in the treatment of early and middle stage chronic renal failure. Methods: 114 patients with early and middle chronic renal failure were randomly divided into two groups, 57 cases in the control group were treated with Urokinase, and 57 cases in the study group were treated with Modified Liuwei Dihuang Decoction combined with Urokinase, and the two groups were treated continuously for 4 weeks. The clinical effect of the two groups after treatment was compared, and the changes of renal function, coagulation function and micro inflammatory state indexes levels in the two groups were detected. Results: After treatment, the total effective rate of the study group was significantly higher than that of the control group (P<0.05);the levels of serum creatinine (SCr), urea nitrogen (BUN), fibrinogen (Fib), serum tumor necrosis factor-α(TNF-α), interleukin-6 (IL-6) and C-reactive protein (CRP) in the two groups were significantly lower than those before treatment (P <0.05), and the above indexes in the study group were significantly lower than those in the control group (P<0.05). The clearance rate of creatinine clearance (CCr), activated partial thromboplastin time (APTT) in the two groups were significantly higher than before treatment (P<0.05), and the above indexes in the study group were significantly higher than those in the control group (P <0.05). Conclusion: Modified Liuwei Dihuang Decoction combined with Urokinase in the treatment of early and middle stage chronic renal failure is effective, and can significantly enhance renal function and coagulation function, and improve microinflammatory state.
