摘  要:目的观察左卡尼汀联合醋酸泼尼松对原发性肾病综合征合并高脂血症患者血脂及肝肾功能的影响。方法选择2017年1月—2018年4月四川省资阳市第一人民医院肾病内科治疗的原发性肾病综合征合并高脂血症患者82例作为研究对象,依据随机数字表法分为对照组(n=41)和联合用药组(n=41)。患者入组后均给予降压、降脂等常规治疗,对照组患者在常规治疗基础上给予醋酸泼尼松,联合用药组患者在对照组治疗基础上给予左卡尼汀。治疗2个月后,对2组患者治疗前后的血脂、肾功能、肝功能指标进行比较=结果治疗后,2组患者的高密度脂蛋白胆固醇均增高,总胆固醇、三酰甘油、低密度脂蛋白胆固醇、血肌酐、血尿素氮、24 h尿蛋白、丙氨酸氨基转氨酶、天门冬氨酸氨基转氨酶均降低,联合用药组的变化幅度均大于对照组(t=-3. 152、3.936、5.229、2.275、2.774、4.084、8. 932,7. 584、11.604,P <0.05,P <0.01)。结论使用左卡尼汀联合醋酸泼尼松治疗原发性肾病综合征合并高脂血症患者,可以降低血脂水平,改善肝肾功能指标,值得临床推广。Objective To observe the effects of L-carnitine combined with prednisone acetate on blood lipids and liver and kidney function in patients with primary nephrotic syndrome and hyperlipidemia. Methods From January 2017 to April 2018, 82 patients with primary nephrotic syndrome complicated with hyperlipidemia treated by nephrology department of the First People’s Hospital of Ziyang City, Sichuan Province, were selected as the study subjects. They were divided into control group(n=41) and combination group(n=41) according to random number table method. Patients in the control group were given prednisone acetate on the basis of routine treatment, while patients in the combined group were given levocarnitine on the basis of the control group. After 2 months of treatment, the blood lipid, renal function and liver function of the two groups were compared before and after treatment. Results After treatment, high-density lipoprotein increased in both groups, total cholesterol, triglyceride, low-density lipoprotein, serum creatinine, blood urea nitrogen, 24 h urine protein, alanine aminotransferase, aspartate aminotransferase The reductions were all greater in the combination group than in the control group(t=-3. 152, t=3.936, t=5.229,t=2.275,t=2.774,t=4.084, t= 8.932, t=7.584, t= 11.604,P <0.05,P <0. 01). Conclusion The use of levocarnitine combined with prednisone acetate in the treatment of primary nephrotic syndrome with hyperlipidemia can reduce blood lipid levels and improve liver and kidney function, which is worthy of clinical promotion.
