两种临时心脏起搏术在急诊经皮冠状动脉介入术中的应用对比

摘　　要：目的对比经静脉临时起搏与经皮体外起搏两种临时心脏起搏术在急诊经皮冠状动脉介入术(PCI)中的应用效果。方法 76例急性心肌梗死(AMI)合并缓慢心室率患者,根据起搏方法不同分为TCP组(40例)和NTCP组(36例)。TCP组患者在急诊PCI中采用经静脉临时起搏, NTCP组患者在急诊PCI中采用经皮体外起搏。比较两组患者起搏疗效、操作时间及术后左室射血分数(LVEF)。结果两组患者起搏成功、临床有效、起搏无效情况比较差异无统计学意义(P>0.05)。NTCP组患者操作时间为(1.03±0.34)min,明显短于TCP组的(13.29±7.31)min,差异有统计学意义(P<0.05)。NTCP组患者术后LVEF为(54±11)%,明显高于TCP组的(49±10)%,差异有统计学意义(P<0.05)。结论急诊PCI中,经皮体外起搏与经静脉临时起搏疗效相当,但经皮体外起搏具有操作时间短的优势,同时可提供电除颤,更为快捷、简便,值得临床推广使用。Objective To compare the effect of temporary temporary pacing of percutaneous external pacing and percutaneous external pacing in emergency percutaneous coronary intervention(PCI).Methods A total of 76 patients with acute myocardial infarction(AMI)with slow ventricular rate were divided by different pacing methods into TCP group(40 cases)and NTCP group(36 cases).TCP group received temporary intravenous pacing in emergency PCI,and NTCP group received extracorporeal pacing in emergency PCI.Comparison were made on pacing efficacy,operation time and left ventricular ejection fraction(LVEF)between the two groups.Results Both groups had no statistically significant difference in successful pacing,clinically effective and ineffective pacing(P>0.05).NTCP group had obviously shorter operation time as(1.03±0.34)min than(13.29±7.31)min in TCP group,and the difference was statistically significant(P<0.05).NTCP group had obviously higher postoperative LVEF as(54±11)%than(49±10)%in TCP group,and the difference was statistically significant(P<0.05).Conclusion In emergency PCI,percutaneous external pacing and transvenous temporary pacing have the same effect,but percutaneous external pacing has the advantage of short operation time,and can provide electric defibrillation.It is faster and simpler and worthy of clinical promotion.
