摘 要:目的:研究缺血性中风病急性期痰热证患者与MMP-2表达的相关性,探讨MMP-2与中风病急性期痰热证是否存在相关性,其相关性研究能否形成微观辨证的依据。方法:筛选发病7天内就诊于中国中医科学院西苑医院的缺血性中风痰热证患者45名,非痰热证44名,由脑病科医师使用《缺血性中风证候要素诊断量表》,对每例患者分别作出内风证、内火证、痰湿证、血瘀证、气虚证和阴虚证的量化诊断,并用ELISA法检测血清中MMP-2的含量。结果:痰热组MMP-2的表达较非痰热组MMP-2表达增强。结论:中风病急性期痰热证的MMP-2的表达明显增高,可能与神经细胞的坏死程度较重有关,该研究以痰热证与基质金属蛋白酶相关亚型的关系为切入点,对痰热证的微观指标提供研究基础,将对中风病痰热证证候诊断及疗效评价提供客观支持,可能为内风证、内火证、痰湿证、血瘀证、气虚证和阴虚证的诊断提供依据。Objective: To study the correlation between the expression of MMP-2 and the patients with phlegm heat syndrome in the acute phase of ischemic stroke, and to investigate whether there is a correlation between MMP-2 and the syndrome of acute spastic fever in the acute phase of stroke, and whether the correlation study can form the basis for microscopic syndromes differentiation. Methods: 45 patients with ischemic apoplexy, phlegm heat syndrome who were treated in Xiyuan Hospital of China Academy of Chinese Medical Sciences within 7 days of onset were screened. 44 patients with non-phlegm heat syndrome were included. The ischemic stroke syndrome diagnostic scale was used by encephalologists. For each patient, the quantitative diagnosis of internal wind syndrome, internal fire syndrome,dampness syndrome, blood stasis syndrome, qi deficiency syndrome, and yin deficiency syndrome was performed, and the MMP2 level in serum was detected by ELISA. Results: The expression of MMP-2 in the phlegm heat group was higher than that in the non-phlegm heat group. Conclusion: The expression of MMP2 in the acute stage of stroke with phlegm heat syndrome was significantly higher and may be related to the severity of necrosis of nerve cells. The relationship between phlegm heat syndrome and MMP-associated subtypes was considered as the starting point for the study of the syndrome of phlegm heat. The micro-indicators provide the basis for research and will provide objective support for the diagnosis and therapeutic evaluation of stroke with phlegm heat syndromes, which may provide evidence for the diagnosis of internal wind syndrome, internal fire syndrome, phlegm dampness syndrome, blood stasis syndrome, qi deficiency syndrome and yin deficiency syndrome.
