Combined therapy of tadalafil and biofeedback in the treatment of functional ejaculatory failure

摘　　要：目的回顾性总结采用他达拉非联合生物反馈治疗功能性不射精症的经验。方法本研究纳入2010年1月至2018年1月间就诊本院的62例患者,中位年龄26.5岁,已排除存在器质性病变的患者。所有患者均接受了心理和行为治疗+小剂量他达拉非规律口服+生物反馈治疗,以治疗后3个月的射精改善情况来判断治疗效果。结果3个月后23例治愈,26例有效,13例未愈。整体来讲治疗有效率79.0%。49例治疗有改善的患者中,29例在治疗后1个月即发生了症状改善。针对治疗效果的影响因素进行分析,发现血糖血脂升高和较高的年龄是影响治疗效果的不良预测因素。结论行为治疗+小剂量他达拉非规律口服+生物反馈治疗治疗功能性不射精症效果良好,高血糖/血脂和高龄是治疗效果不佳的危险因素。Objective To demonstrate the initial experience of treating functional ejaculatory failure by tadalafil and biofeedback. Methods From January 2010 to January 2018, 62 patients were included in the study with a median age of 26.5. Patients with organic pathologic changes were excluded. All patients received psychotherapy and behavior therapy, small-dose tadalafil and biofeedback treatment. Treatment results were defined by patients'status after 3 months. Results Treatment was effective in 49 patients, including 23 patients who were cured, and the general effective rate was 79.0%. Twenty-nine patients experienced improvement after 1 month. Elevated blood glucose and hyperlipidemia and older age were risk factors for unsatisfactory treatment outcomes. Conclusions Combined behavior therapy, small-dose tadalafil and biofeedback treatment was effective in treating patients with function ejaculatory failure. Elevated blood glucose and hyperlipidemia and older age were risk factors for unsatisfactory treatment outcomes.
