摘  要:目的:探讨320排CT可变螺距法在主动脉夹层(AD)成像及诊断中的应用价值。方法:纳入31例AD患者分为两组,其中16例以可变螺距法扫描作为VHP组,主动脉弓上缘至膈面水平采用标准螺距加心电门控扫描,膈面至耻骨联合水平采用大螺距非心电门控扫描。15例以非心电门控大螺距全主动脉扫描作为对照组。比较两组的图像质量、辐射剂量及诊断价值。结果:两组图像不同兴趣区的CT值和噪声标准差无明显差异(P>0.05)。VHP组有效剂量(14.33±1.01) mSv,稍高于对照组为(13.76±0.95) mSv,但两组间差异无统计学意义(P>0.05)。VHP组图像对内膜破口、主动脉瓣、内膜片及真假腔的可诊断率达到100%,明显高于对照组。结论:对于AD患者,相对于传统非心电门控法,VHP扫描法可以在不明显增加辐射剂量的前提下消除内膜破口、主动脉瓣、内膜片的伪影,具有较好的临床应用价值。Purpose: To explore the value of variable helical pitch(VHP) method of 320-slice volume CT in diagnosing aortic dissection. Methods: Thirty-one patients with acute aortic syndrome were consecutively enrolled and randomly divided into two groups. There were 16 patients in the VHP group, the standard pitch with ECGgating CT angiography were performed from the upper edge of the aortic arch to the bottom of the heart, while big pitch without ECG-gating were performed from the bottom of the heart to the level of symphysis pubis. The other 15 patients who were scanned by big pitch without ECG-gating for the whole aorta were treated as the control group.The image quality, radiation dose and diagnostic value between two groups were compared, and statistical analysis was performed. Results: There was no significant statistical difference in CT value and noise standard deviation between two groups of images(P>0.05). The effective dose(14.33±1.01) mSv in the VHP group was slightly higher than that in the control group(13.76±0.95) mSv, but there was no significant statistical difference between the two groups(P>0.05).The diagnosis rate of the aortic valve, intimal tear, true and false lumen in the VHP group was higher than those in the control group. Conclusion: For patients with aortic dissection, compared with the traditional non ECG gated method, the VHP method can eliminate the artifacts of the aortic valve, intimal tear, aortic intimal without obvious increase of radiation dose, it has high clinical application value.
