2015—2017年上海某医院门诊药房退药情况分析

摘　　要：目的 :分析门诊药房退药情况,为减少不合理退药提供依据。方法 :资料来源于松江区中心医院HIS系统2015年1月1日—2017年12月31日门诊处方调配记录及医院门诊退药登记簿。对退药处方的金额、退药药品种类及退药原因等进行分析,并对退药原因进行帕累托图分析。结果 :2015—2017年,门诊退药处方构成比分别为1.88‰、2.25‰、0.78‰;退回药品种类主要以抗菌药物、中成药、消化系统、心脑血管系统和呼吸系统药物为主;门诊药房退药原因3年间有所变化,共同的主要因素有不良反应、重复开药、医保经济因素、依从性差及家中有药。结论 :退药原因涉及到患者、医师和药房,需对不同因素进行深入分析,细化改进措施,才能减少退药的发生。Objective: To analyze drug repercussion in the out-patient pharmacy for providing a basis for reducing unreasonable drug repercussion. Methods: The data came from the outpatient prescription allocation record in the HIS system of Songjiang District Central Hospital and the hospital outpatient drug withdrawal registration book from January 1, 2015 to December 31, 2017. Data analysis was made on the amount of the prescription for drug repercussion, the types of drug repercussion and the reasons for the drug repercussion, and Pareto diagram analysis was made on the reasons for the drug repercussion. Results: From 2015 to 2017, the rates of drug repercussion were 1.88‰, 2.25‰ and 0.78‰, respectively;the types of drug repercussion were mainly drugs of antibiotics, Chinese patent medicine, digestive, cardiovascular and respiratory systems;the reasons for drug repercussion had changed in 3 years, but their common main factors for drug repercussion were ADR, repeated prescription, economic factors of medical insurance, poor compliance and having drugs at home. Conclusion: The reasons for drug repercussion involve patients, physicians and pharmacies, and it is necessary to conduct in-depth analysis of different factors and refine the improvement measures in order to reduce the occurrence of drug repercussion.
