摘 要:目的利用特异性CT靶向对比剂对活体动脉粥样硬化动物模型的斑块内巨噬细胞进行成像与量化,并探讨巨噬细胞定量与斑块易损性的关系。方法应用靶向对比剂对活体动物模型斑块内巨噬细胞进行CT成像,以斑块CT值50HU为界点,分为易损与非易损斑块组,测量斑块面积(plaque area, PA)、斑块负荷(plaque volume, PB)、管腔狭窄率(lumen stenosis rate, LSR)、重构指数(remodeling index, RI)、斑块内对比剂蓄积区高密度影面积(high density area, HDA),并计算HDA/PA值。结果 1)易损斑块组PA(2.52±0.83)、PB(0.68±0.15)、RI(1.13±0.06)、HDA/PA(0.24±0.15)量化值均较非易损斑块组大,差异具有统计学意义(P<0.05);LSR差异无统计学意义(P>0.05);2)PA、PB、RI、HDA/PA均为易损斑块的风险因素。结论基于靶向对比剂的CT成像技术,能够在活体内实现对于粥样斑块内巨噬细胞的成像及量化,进一步拓展了影像学技术在动脉粥样硬化性疾病的早期预警及易损性预测的应用价值。Objective To achieve the imaging and quantification of macrophages in plaques of living atherosclerotic animal models using specific CT targeting contrast agents and to investigate the relationship between the quantification of macrophages and plaque vulnerability. Methods Firstly, targeted contrast agents were used for CT imagings of macrophages in plaques of living animal models. After that,through setting 50 HU as the critical CT value, all the plaques were divided into two groups, the vulnerable and non vulnerable plaque group. Plaque area (PA), plaque volume (PB), lumen stenosis rate (LSR), remodeling index (RI), high density area (HDA) were measured,respectively. Finally, the values of HDA/PA were calculated. Results 1) All the values of PA(2.52±0.83), PB(0.68±0.15), RI(1.13±0.06), HDA/PA(0.24±0.15) in the vulnerable plaque group were higher than those in non vulnerable plaque group, and these differences were statistically significant ( P <0.05). The difference of LSR was not statistically significant ( P >0.05);2)PA, PB, RI and HDA/PA were all the risk factors of vulnerable plaques. Conclusion Based on the CT imaging using targeted contrast agents, the imaging and quantification of macrophages in plaques of living animals can be achieved,and the application of imaging techniques in the early warning and vulnerability evaluation of atherosclerotic diseases can be further developed well.
