摘　　要：目的:探讨早期鼻空肠营养联合生大黄胃内灌注治疗重症急性胰腺炎的有效性。方法:选取58例重症急性胰腺炎患者,随机分为两组,实验组给予鼻空肠管置管,并在当天给予生理盐水250 mL鼻肠管匀速泵入,若患者无不适症状,逐渐匀速给予肠内营养液,同时给予大黄浸液胃内灌注。对照组给予肠外营养支持,并持续胃肠减压。结果:实验组在肠功能恢复,腹痛腹胀缓解,白细胞、血淀粉酶恢复正常时间短于对照组。结论:经鼻空肠管肠内营养支持联合大黄胃内灌注治疗重症急性胰腺炎的效果优于全肠外营养治疗。Objective: To explore the efficacy of early nasojejunal nutrition combined with gastric infusion of rhubarb for the treatment of severe acute pancreatitis(SAP). Methods: A total of 58 patients with SAP were randomly divided into two groups:The treament group and the control group. The treatment group was indwelled a nasal jejunal tube, and then infused of 250 mL normal saline. If patients had no symptoms, the patients were gradually given the inner nutrient, and simultaneously infused of rhubarb extract through gastric tube. The control group was given parenteral nutrition support and continued gastrointestinal decompression. Results: In the experimental group, the recovery time of intestinal function and relief time of abdominal pain and distension were shorter than that of the the control guoup. In the experimental group the abnormal white blood cell counts and level of plasma amylase were restored to normal in shorter time than that of the control group. Conclusions: Compared to the total parenteral nutrition, combination of enteral nutrition and rhubarb intragastric infusion can significantly improve severe acute pancreatitis patients.
