摘 要:目的:观察人脐带沃顿胶薄片对大鼠面神经损伤后髓鞘的修复效果及可能机制。方法:64只成年雌性Wistar大鼠随机分成假手术组(n=14)、模型组(n=25)及沃顿胶薄片组(n=25),后2组建立面神经挤压伤模型,其中,沃顿胶薄片组将人脐带沃顿胶薄片贴附于面神经挤压伤处。分别于造模后第1、3、10、20、30天评价面神经功能。造模后第30天测定面神经电生理。取损伤处面神经主干,采用HE染色观察病理改变,透射电镜观察超微结构; Western blot检测髓鞘碱性蛋白(MBP)、脑源性神经营养因子(BDNF)及神经生长因子(NGF)的表达; ELISA测定炎症因子IL-1β、IL-10、TNF-α、NF-κB的分泌。结果:造模后第1、3、10、20、30天,模型组面神经功能评分均较假手术组升高;第20、30天,沃顿胶薄片组面神经功能评分较模型组降低(P <0. 05)。造模后第30天,模型组M波潜伏期升高,神经电位波幅降低,沃顿胶薄片组以上2指标均较模型组改善(P <0. 05); HE染色显示模型组面神经主干有广泛的脱髓鞘改变,而沃顿胶薄片组面神经组织结构恢复,可见再生的髓鞘;透射电镜下可见模型组神经纤维肿胀,呈现脱髓鞘改变,而沃顿胶薄片组髓鞘明显增厚,髓鞘板层结构松散明显好转,空泡状改变减少。与模型组相比,沃顿胶薄片组MBP、BDNF及NGF的表达增多,IL-1β、IL-10、NF-κB的表达降低(P <0. 05)。结论:人脐带沃顿胶薄片可促进大鼠面神经损伤后的髓鞘修复,其机制可能与沃顿胶中MBP促进神经营养因子的分泌,减少炎症因子的释放,从而改善损伤区域局部微环境有关。Aim:To observe the effects of human umbilical cord Wharton gel slice on myelin sheath repair after facial nerve injury and its possible mechanism.Methods:Sixty-four adult female Wistar rats were randomly allocated into sham operation group(n=14),model group(n=25)and Wharton gel slice group(n=25).The latter 2 groups were made nerve crush injury model,and in the Wharton gel slice group the human umbilical cord Wharton gel slice was attached to the facial nerve crush injury.On the 1st,3rd,10th,20th,and 30th day after modeling,the facial nerve function was evaluated.On the 30th day,the facial nerve electrophysiology was measured.The pathological changes of the facial nerve were observed by HE staining,and the ultrastructure was observed by transmission electron microscopy.The expressions of myelin basic protein(MBP),brain derived neurotrophic factor(BDNF)and nerve growth factor(NGF)in the lesion were detected by Western blot.The secretion of inflammatory factors IL-1β,IL-10,TNF-αand NF-κB in the lesion was determined by ELISA.Results:On the 1st,3rd,10th,20th and 30th day after modeling,the facial nerve function scores of the model group increased compared with sham operation group;on the 20th and 30th day,the scores of the Wharton gel slice group decreased compared with the model group(P<0.05).On the 30th day after modeling,the latency of M wave increased while the amplitude decreased in the model group,and the 2 indexes in the Wharton gel slice group both improved compared with the model group(P<0.05).HE staining showed extensive demyelination in the model group,the facial nerve structure restored and the regenerative myelin sheath was observed in the Wharton gel slice group.Under the transmission electron microscope,the demyelination could be seen in the model group,while the myelin sheath of the Wharton gel slice group was thickened,the structure of the myelin sheath layer was improved,and the vacuole-like change was less.Compared with the model group,the expressions of MBP,BDNF and NGF in the Wharton gel slice
