S1AI与S2AI螺钉内固定对腰骶段脊柱结核的治疗效果比较

摘　　要：目的比较经骶1骶髂(S1 alar iliac,S1AI)螺钉与经骶2骶髂(S2 alar iliac,S2AI)螺钉内固定对腰骶段脊柱结核的治疗效果。方法选取2014年3月至2017年2月本院收治的80例腰骶段脊柱结核患者为研究对象,采用随机数表法将其分为观察组(40例)和对照组(40例),观察组患者采用S1AI螺钉内固定,对照组患者采用S2AI内固定,随访12个月,比较两组患者手术前后红细胞沉降率、降钙素原水平、视觉模拟评分法(visual analogue scale,VAS)评分、Oswestry功能障碍指数(Oswestry disability index,ODI)评分、并发症发生率及术后翻修率。结果术后7 d,两组患者红细胞沉降率、降钙素原水平、VAS评分、ODI评分均显著低于本组术前(P均< 0.05),且观察组患者ODI评分和翻修率均显著低于同期对照组(P均<0.05)。两组患者术后VAS评分和并发症发生率比较均无显著差异(P均> 0.05)。结论 S2AI螺钉内固定和S1AI螺钉内固定对腰骶段脊柱结核患者的治疗效果相当,但S1AI螺钉内固定能够降低患者翻修率,减轻患者功能障碍。Objective To compare the therapeutic effects of S1 alar iliac (S1AI) screw and S2 alar iliac (S2AI) screw fixation in the treatment of lumbosacral spinal tuberculosis. Method From March 2014 to February 2017, 80 patients with lumbosacral spinal tuberculosis in our hospital were randomly divided into observation group (n = 40) and control group (n = 40), the patients in observation group were treated with S1AI screw internal fixation, the patients in control group were treated with S2AI. All patients were followed up for 12 months, erythrocyte sedimentation rate, procalcitonin level, visual analogue scale (VAS) score, Oswestry disability index (ODI) score, incidence of complications and postoperative revision rate were compared between the two groups. Result 7 days after operation, the erythrocyte sedimentation rate, procalcitonin level, VAS scores and ODI scores in the two groups were significantly lower than those before operation (Pall < 0.05), the ODI score and revision rate in observation group were significantly lower than those in control group (Pall < 0.05). There were no significant differences in postoperative VAS scores and incidence of complications between the two groups (Pall > 0.05). Conclusion S2AI and S1AI screw internal fixation have the same effect in the treatment of lumbosacral spinal tuberculosis, but S1AI screw internal fixation can reduce the revision rate and the dysfunction of lumbosacral spinal tuberculosis.
