摘 要:目的分析丙戊酸钠鼻饲和口服给药对其血药浓度影响的差异性,以及血药浓度与给药剂量的相关性。方法回顾性收集南京医科大学附属苏州医院2016年1月至2017年12月期间单药使用丙戊酸钠片的病例,分为鼻饲给药组和口服给药组。记录患者的基本信息、丙戊酸钠的给药剂量和给药方式、给药后测得丙戊酸钠的稳态血药浓度等。结果实际纳入病例258例,包括鼻饲组136例和口服组122例。患者丙戊酸钠血药浓度与日剂量作相关性分析显示两者呈正相关(鼻饲组R=0.617,P<0.001;口服组R=0.435,P<0.001)。当给予患者丙戊酸钠日剂量600 mg和1 200 mg时,鼻饲组血药浓度均明显低于口服组[(31.0±10.4)μg/ml比(44.2±13.9)μg/ml,P<0.001;(57.5±22.5)μg/ml比(62.0±9.8)μg/ml,P=0.003)]。结论丙戊酸钠血药浓度与日剂量呈正相关,相同给药剂量时丙戊酸钠鼻饲给药方式血药浓度明显低于口服组。Objective To analyze the differences of serum concentrations of sodium valproate by nasogastric and oral administration, and the correlation between serum concentration and dosage. Methods Patients taken single-agent sodium valproate tablets were collected retrospectively from January 2016 to December 2017 from The Affiliated Suzhou Hospital of Nanjing Medical University. The patients were divided into a nasogastric administration group and an oral administration group. The basic information as well as the dosage, administration method, and the serum concentration of sodium valproate were recorded. Results 258 cases were included, with 136 in the nasogastric group and 122 in the oral group. The linear correlation analysis indicated that there was a positive correlation between the serum concentration and daily dose of sodium valproate (The nasogastric group: R=0.617, P<0.001;the oral group: R=0.435, P<0.001). The serum concentrations at the daily doses of 600 mg and 1 200 mg were significantly lower in the nasogastric group than in the oral group [(31.0±10.4)μg/ml vs.(44.2±13.9)μg/ml, P<0.001;(57.5±22.5)μg/ml vs.(62.0±9.8)μg/ml, P=0.003]. Conclusion There is a positive correlation between the serum concentration and daily dose of sodium valproate. At the same dosage, the serum concentration of sodium valproate was significantly lower in the nasogastric group than in the oral group.
