PHILOS自锁系统在治疗中老年肱骨近端骨折患者中的优势

摘　　要：目的对比肱骨近端内锁定系统(PHILOS)和肱骨近端锁定钢板(LPHP)治疗中老年骨折的疗效,分析PHILOS术的优势。方法回顾分析梅州市五华县人民医院自2014年1月至2017年12月期间,90例(55~85岁)接受肱骨近端骨折治疗的患者资料,分为PHILOS组(n=45)和LPHP组(n=45),术后分析Neer肩关节功能评分、Constant-Murley肩关节评分(CMS)、直观类比量表评分(VAS)、肱骨头内翻角度和并发症。结果对比LPHP组,PHILOS组的VAS评分和并发症发生率较低、CMS评分和Neer评分优良率较高、肱骨头内翻角度> 5°的人数较少,差异具有统计学意义(P <0.05)。结论 PHILOS术治疗中老年(≥55岁)肱骨近端骨折的疗效优于LPHP术,PHILOS术有助于降低患者疼痛感、促进关节修复、减少并发症。Objective To compare the effect of proximal humerus locking system(PHILOS)and locking proximal humerus plate( LPHP) in treating proximal humeral fractures. Methods Data of 90 cases( aged from 55 to 85) with proximal humeral fractures from January of 2014 to December of 2017 in People′s Hospital of Wuhua County were divided into PHILOS group(n = 45) and LPHP group(n = 45). The therapeutic effect was evaluated by Neer system,Constant-Murley System( CMS), visual analog scale( VAS) score,varus of humerus and complications.Results Comparing to those in LPHP group,V AS score and complication rate in PHILOS group was lower,while scores of CMS and Neer were higher and there were less patients with varus of humerus > 5°. All the differences were statistically significant(P < 0.05). Conclusion PHILOS is superior to LPHP in treating proximal humeral fractures patients ≥ 55 years old,which is helpful for reducing pain,improving articulatio humerus repair and decreasing complications rate.
