摘　　要：目的探讨A群链球菌热灌注治疗对经尿道电切术后高危型腺性膀胱炎患者排尿功能的影响。方法选取2015年1月～2016年12月在我院治疗的40例高危型腺性膀胱炎患者为研究对象,并随机分为试验组(n=20)和对照组(n=20)。两组均采取经尿道膀胱肿瘤电切术(TURBT)治疗,术后试验组行A群链球菌热灌注治疗,术后随访12个月。记录化疗过程中试验组患者出现的不良反应,通过膀胱镜检查及OABSS量表评定两组患者复发情况,比较两组术后排尿功能的差异。结果试验组所有患者均未出现因骨髓抑制、尿道狭窄、膀胱挛缩等不良反应而停止灌注治疗。术后第9和第12个月试验组患者OABSS评分低于对照组(P<0.05)。试验组术后复发率低于对照组(P<0.05)。从术前至术后第12个月,两组排尿次数和残余尿量逐渐减少,最大尿流率逐渐升高(P<0.05);且与对照组比较,术后第3、6、12个月试验组排尿次数和残余尿量减少,最大尿流率升高(P<0.05)。结论 A群链球菌热灌注治疗TURBT术后高危型腺性膀胱炎可改善患者排尿功能,降低复发率,且安全性较好。Objective To investigate the effect of Strep-A hyperthermic perfusion chemotherapy on micturition function of patients with high-risk cystitis glandularis after transurethral resection.Methods 40 cases with high-risk cystitis glandularis treated in our hospital from Jan 2015 to Dec 2016 were selected and randomly divided into the experimental group and the control group,20 cases in each group.Transurethral resection of bladder tumor (TURBT) was performed in the two groups,and Strep-A hyperthermic perfusion was carried on the experimental group after operation.The two groups were followed up for 12 months after operation.The adverse reaction of the experimental group was observed during the chemotherapy.The overactivity of bladder symptom score (OABSS) and cystoscopy were performed in the two groups,and the recurrence was observed,the postoperative micturition function was compared between the two groups.Results All patients in the experimental group did not have adverse reactions such as bone marrow suppression,urethral stricture and bladder contracture.The OABSS score of the experimental group was lower than that of the control group at the ninth and twelfth month after operation (P< 0.05).The total recurrence rate of the experimental group within 12 months after operation was lower than that of the control group (P< 0.05).From preoperation to the twelfth month after operation,the time of urination and residual urine volume in the two groups gradually decreased,and the maximum urine flow rate gradually increased (P< 0.05),and the number of urination and residual urine volume in the experimental group were lower than those of the control group,the maximum urine flow rate was higher than that of the control group at the third,sixth,twelfth month after operation (P< 0.05).Conclusion The Strep-A hyperthermic perfusion chemotherapy can reduce the recurrence rate of patients with high-risk cystitis glandularis after TURBT and improve the micturition function,and the safety is good,the treatment effect is remarkable.
