变应性鼻炎患儿血清YKL-40水平与体液免疫功能的变化及关系分析

摘　　要：目的探讨变应性鼻炎(AR)患儿血清几丁质酶-3样蛋白-1(YKL-40)水平与体液免疫功能的变化,并分析两者间的关系。方法选取2017年1月～2018年1月我院收治的158例AR患儿为研究对象,根据病情严重程度分为轻度组(轻度AR,71例)和中重度组(中重度AR,87例),另将同期健康儿童60例纳为对照组,测定对照组及AR患儿治疗前后血清YKL-40、免疫球蛋白A(IgA)、免疫球蛋白E(IgE)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)的水平,并分析血清YKL-40水平与IgA、IgE、IgG、IgM的相关性。结果 AR组血清YKL-40、IgA、IgE、IgG、IgM水平均明显高于对照组(P<0.05);中重度组血清YKL-40、IgA、IgE、IgG、IgM水平均明显高于轻度组(P<0.05);AR患儿治疗后血清YKL-40、IgA、IgE、IgG、IgM水平均明显低于治疗前(P<0.05);血清YKL-40水平与IgA、IgE、IgG、IgM均呈正相关(P<0.05)。结论 AR患儿血清YKL-40异常高表达,其表达量与患儿病情严重程度及体液免疫功能有关。Objective To explore the changes of serum YKL-40 level and humoral immune function in children patients with allergic rhinitis (AR),and to analyze the relationship between them.Methods A total of 158 children patients with AR admitted to our hospital from Jan 2017 to Jan 2018 were selected for the study.According to the disease severity,they were divided into mild group (mild AR,n=71) and moderate-to-severe group (moderate-to-severe AR,n=87).Another 60 cases of healthy children were included in the control group.The levels of serum YKL-40,IgA,IgE,IgG and IgM were measured before and after treatment in 3 groups.The correlation between serum YKL-40 level and IgA,IgE,IgG,IgM was analyzed.Results The levels of serum YKL-40,IgA,IgE,IgG and IgM in AR group were significantly higher than those in the control group (P< 0.05).The levels of serum YKL-40,IgA,IgE,IgG and IgM in moderate-to-severe group were significantly higher than those in mild group (P< 0.05).The levels of serum YKL-40,IgA,IgE,IgG and IgM in children patients with AR after treatment were significantly lower than those before treatment (P< 0.05).Serum YKL-40 level was positively correlated with IgA,IgE,IgG and IgM (P< 0.05).Conclusion The serum YKL-40 expression is abnormally high in children patients with AR,and its expression level is related to the disease severity and humoral immune function.
