基于微信平台的延续性护理对中青年CHF患者自我管理能力的影响

摘　　要：目的:探讨基于微信平台的延续性护理对中青年慢性心力衰竭(CHF)患者自我管理能力及再入院率的影响。方法:将治疗出院的100例中青年CHF患者按照住院号的奇偶数分为观察组和对照组各50例,对照组采用常规的延续性护理(出院指导+电话随访),观察组在常规延续性护理基础上利用微信平台进行延续性护理,为期6个月;观察并比较两组患者于出院后1、3、6个月的自我管理能力,并统计6个月内的再入院率。结果:出院后1、3、6个月,观察组心力衰竭自我管理量表各维度评分明显优于对照组(P <0. 05);两组患者6个月内再入院率比较差异有统计学意义(P <0. 01)。结论:基于微信平台的延续性护理可明显提高中青年CHF患者的自我管理能力,降低再入院率,值得临床推广。Objective:To explore the effect of continuing care based on WeChat platform on the self-management ability and readmission rate of young and middle-aged patients with chronic heart failure(CHF).Methods:A total of 100 young and middle-aged CHF patients who were discharged from the hospital were divided into the observation group and the control group with 50 cases each group according to the odd even number of admission Numbers.The control group received routine continuous nursing(discharge guidance+telephone follow-up).The observation group was given routine continuous nursing on the basis of routine continuous nursing using the WeChat platform for 6 months.The self-management ability of patients in the two groups at 1,3 and 6 months after discharge was observed and compared,and the readmission rate within 6 months was calculated.Results:In 1,3 and 6 months after discharge,the scores of each dimension of the self-management scale of heart failure in the observation group were significantly better than those in the control group( P <0.05).There was a statistically significant difference in the readmission rate between the two groups within 6 months( P <0.01).Conclusion:Continuous nursing based on WeChat platform can significantly improve the self-management ability of young and middle-aged patients with CHF and reduce the readmission rate,which is worthy of clinical promotion.
