甲状腺癌根治术与甲状旁腺自体移植术联合对甲状腺癌的应用及疗效分析

摘　　要：目的观察甲状腺癌根治术与甲状旁腺自体移植术联合对甲状腺癌的应用及疗效。方法以2017年8月—2018年6月作为观察时间段,方便选取此时间段内收治的甲状腺癌患者480例,分为参照组(甲状腺癌根治术)和联合组(在参照组上联合甲状旁腺自体移植术)。观察两组患者的临床疗效。结果联合组及参照组患者术后3个月血清甲状腺激素水平比较,差异有统计学意义(t=24.615 6,P=0.000 0);术后半年组间比较(t=20.446 3,P=0.000 0);联合组短暂性发生率10.42%、参照组发生率17.50%,差异有统计学意义(χ~2=5.013 2,P=0.025 2);联合组永久性甲状旁腺功能减退发生率7.50%低于参照组16.25%,差异有统计学意义(χ~2=8.779 4,P=0.003 0)。结论对甲状腺癌患者应用甲状腺癌根治术联合甲状旁腺自体移植术,患者短暂性及永久性甲状旁腺功能减退发生事件少,血清甲状腺素水平明显改善,值得临床推荐。Objective To observe the application and efficacy of thyroid cancer radical surgery combined with autologous transplantation of parathyroid gland in thyroid cancer. Methods A total of 480 patients with thyroid cancer admitted to this period were convenient selected from August 2017 to June 2018. They were divided into reference group (radical thyroidectomy) and combined group (with thyroid in the reference group combined with paragonadal autograft). The clinical efficacy of the two groups of patients was observed. Results The serum thyroid hormone levels in the combined group and the reference group were compared at the three months after surgery,the different was statistically significant(t=24.615 6, P=0.000 0). After the half-year group comparison(t=20.446 3, P=0.000 0);the transient incidence rate of the combined group was 10.42%. The incidence rate of the reference group was 17.50%,the different was statistically significant(χ^2=5.013 2, P=0.025 2);the incidence of permanent hypoparathyroidism in the combined group was 7.50% lower than that in the reference group, 16.25%,the different was statistically significant(χ^2=8.779 4, P=0.003 0). Conclusion In patients with thyroid cancer, thyroidectomy combined with parathyroid autograft transplantation, patients with transient and permanent hypoparathyroidism have fewer events, serum thyroxine levels improved significantly, it is worthy of clinical recommendation.
