高危型人乳头瘤病毒感染与阴道常见微生物感染的相关性研究

摘　　要：目的探讨高危型人乳头瘤病毒(HR-HPV)感染与阴道常见微生物感染的相关性。方法收集2018年1月至4月就诊于北京市海淀医院妇科门诊,同时检测阴道分泌物常规、支原体培养及HR-HPV分型的1999例患者的临床资料并进行分析。结果1999例患者中,HR-HPV阳性组434例,占21.71%(434/1999),HR-HPV阴性组1565例,占78.29%(1565/1999)。细菌性阴道病(BV)共63例,占3.15%(63/1999),其中HR-HPV阳性组中BV的构成比为4.84%,HR-HPV阴性组中BV的构成比为2.68%,两组BV的构成比比较,差异有统计学意义(P<0.05)。BV与HR-HPV感染呈正相关(OR=1.84,95%CI为1.08~3.15,P=0.02),而HR-HPV感染与外阴阴道假丝酵母菌病(VVC)、滴虫性阴道炎(TV)和解脲支原体(UU)、人型支原体(MH)的感染及阴道混合感染无相关性(P>0.05)。结论BV与HR-HPV感染呈正相关,而VVC、TV、UU、MH及阴道混合感染与HR-HPV感染无关。Objective To explore the correlation between high-risk human papillomavirus(HR-HPV)and common vaginal pathogen infections.Methods Clinical data from 1999 gynecologic outpatients in Beijing HaiDian Hospital,who underwent the vaginal secretion routine,mycoplasma culture and HR-HPV type detection from January,2018 to April,2018,were collected and analyzed.Results Among 1999 patients,434 were HR-HPV positive,with the positive rate 21.71%(434/1999),and 1565 were HR-HPV negative,with the negative rate 78.29%(1565/1999).BV was found in 63 cases(3.15%,63/1999).The proportion of BV was 4.84% in the HR-HPV positive group and 2.68% in the HR-HPV negative group.The proportion of BV was significantly different between the two groups(P<0.05).The intensity of HR-HPV infection was positively correlated with BV( OR =1.84,95% CI 为1.08-3.15,P =0.02),while VVC,TV,UU and MH and mixed infection were uncorrelated with HR-HPV infection(allP>0.05).Conclusion BV is positively correlated with HR-HPV infection.VVC,TV,UU and MH and mixed infection are not correlated with HR-HPV infection.
