DAA入路全髋关节置换术治疗青壮年晚期股骨头坏死

摘　　要：目的:观察DAA入路全髋关节置换术治疗青壮年晚期股骨头坏死的临床疗效。方法:自2016年4月-2017年12月,对26例34髋青壮年晚期股骨头坏死患者实施DAA入路全髋关节置换术,术后进行3个月以上的随访,髋关节功能评分选用Harris髋关节评分量表,根据评分结果评估患者治疗效果。结果:本组25例33髋获得随访。随访时间3-25个月,中位随访时间5个月。Harris评分:术前平均52分,术后末次随访髋关节功能Harris评分89-100分,平均95分,同术前相比差异有统计学意义(P<0.01)。术后能实现早期下地行走甚至深蹲等。结论:DAA入路全髋关节置换术治疗青壮年晚期股骨头坏死可以获得更好的早期疗效,而长期疗效与传统全髋关节置换术相当,是青壮年股骨头坏死晚期治疗的良好选择。Objective: To observe the clinical effect of DAA for total hip arthroplasty on the treatment of advanced osteonecrosis of the femoral head in young and middle 一 aged adults. Methods: From April 2016 to December 2017 , 26 patients with advanced osteonecrosis of the femoral head in 34 young and middle 一 aged hips were treated with direct anterior total hip replacement, followed up for more than 3 months after surgery. Harris hip score scale was used to evaluate the therapeutic effect of patients according to the scoring. Results: In this group, 25 patients with 33 hips were followed up. The follow - up time was 3 ~ 25 months, and the median follow - up time was 5 months. Harris score: the preoperative Harris scores was 52 in average the average score of 89 ~ 100 points in the last postoperative follow -up was 95 points, and the difference was statistically significant ( P < 0. 01 ) compared with that before surgery). After surgery can be early implementation of ground walking and even squat. Conclusion: DAA for total hip arthroplasty can obtain better early curative effect, and the long 一 term curative effect is similar to that of traditional total hip arthroplasty, which is a good choice for advanced treatment of young and middle - aged femoral head necrosis.
