住院老年吞咽障碍病人衰弱现况分析

摘　　要：[目的]探讨住院老年吞咽障碍病人衰弱现况,为老年吞咽障碍病人的康复护理提供参考依据。[方法]2016年9月—2018年3月在宁夏某三级甲等医院康复科住院的118例老年吞咽障碍病人,采用吞咽障碍评价标准及Fried衰弱表型评估方法对病人进行评估。[结果]老年吞咽障碍病人衰弱发生率为49.2%,衰弱前期发生率为40.7%,不同吞咽障碍程度病人衰弱状态、体重下降、自述疲乏及握力下降比较,差异具有统计学意义。[结论]老年吞咽障碍病人衰弱及衰弱前期的发生率均较高,不同吞咽障碍程度可能与衰弱状态、体重下降、疲乏及握力下降发生有关。在临床工作中,康复护士应关注吞咽障碍病人衰弱状态,积极改善病人吞咽功能,从而减少负性事件的发生率。Objective: The aim of this study was to investigate the frailty status quo of hospitalized elderly patients with dysphagia,and to provide a reference for rehabilitation nursing on them. Methods: From September 2016 to March 2018,118 elderly patients with dysphagia who was admitted to the rehabilitation department of a Tertiary Grade A hospital in Ningxia city were enrolled in this study.They were assessed by dysphagia evaluation criteria and Fried’s Frailty Phenotype (FP) assessment method. Results: The incidence of debilitating in elderly patients with dysphagia was 49.2%,and the incidence of pre-debilitating was 40.7%.The debilitating status,decrease of body mass index (BMI),self- reported fatigue,and decrease of grip strength among patients with different degree of dysphagia had a statistical significance. Conclusions: The incidence of debilitating and pre-debilitating in elderly patients with dysphagia was high.The different degree of dysphagia might be related to the debilitating status,decrease of BMI,fatigue,and decrease of grip strength.In clinical work,rehabilitation nurses should pay attention to the debilitating status of patients with dysphagia,and actively improve the swallowing function of patients,thereby reducing the incidence of negative events.
