急性心肌梗死合并心律失常Lorenz散点图的形态特征及临床意义

摘　　要：目的探究急性心肌梗死合并心律失常Lorenz散点图的形态特征及临床意义。方法择取2016年1月—2018年1月该院收治的80例急性心肌梗死合并心律失常患者作为研究组,并择取同期健康体检者80名作为对照组,所选研究对象均行以动态心电图检查,绘制Lorenz散点图,分析和对比形态特征。结果从Lorenz散点图形态特征上来看,研究组三分布形占比为30.0%、四分布形占比25.0%、扇形占比12.5%、不规则形占比16.3%、短棒状形占比8.8%、彗星状形占比为7.5%,对照组三分布形占比为2.5%、四分布形占比2.5%、扇形占比0.0%、不规则形占比0.0%、短棒状形占比0.0%、彗星状形占比为90.0%,可见研究组除彗星状形占比低于对照组之外,其他形态占比均高于对照组(P<0.05)。急性心肌梗死合并不同类型心律失常的Lorenz散点图形态如下:无心律失常患者呈彗星状形;合并阵发性室性早搏及房性早搏患者呈三分布形;合并频发性房性早搏及室性早搏患者呈四分布形;合并频发性多元性房性早搏及室性早搏、Ⅱ～Ⅲ度房室阻滞、短阵房性心动过速患者呈不规则形;合并心房颤动患者呈扇形;合并阵发性室性心动过速患者呈短棒状形。结论对于急性心肌梗死合并心律失常来说,Lorenz散点图形态特征分析可以为临床诊断提供依据,有一定临床价值。Objective To investigate the morphological features and clinical significance of Lorenz scatter plots in patients with acute myocardial infarction complicated with arrhythmia. Methods Eighty patients with acute myocardial infarction and arrhythmia admitted to our hospital from January 2016 to January 2018 were enrolled as the study group,and 80 healthy subjects were selected as the control group. All the selected subjects were selected. Dynamic electrocardiography, mapping Lorenz scatter plots, analyzing and contrasting morphological features. Results From the morphological characteristics of Lorenz scatter plot, the proportion of the three distributions in the study group was 30.0%,the proportion of the four distributions was 25.0%, the proportion of the sector was 12.5%, the proportion of the irregulars was 16.3%, and the proportion of the short rods was 8.8%, the proportion of comet shape was 7.5%, the proportion of the three distributions of the control group was 2.5%, the proportion of the four distributions was 2.5%, the proportion of the sector was 0.0%, the proportion of the irregular shape was 0.0%, and the proportion of the short rods was0.0% and comet-like forms was 90.0%. It can be seen that the proportion of other forms was higher in the study group than in the control group(P<0.05). The Lorenz scatter plot of acute myocardial infarction with different types of arrhythmia is as follows: patients with arrhythmia have a comet-like shape;patients with paroxysmal ventricular premature beats and atrial premature beats have a three-distribution shape;combined with frequent atrial premature beats and patients with ventricular premature beats have a four-distribution shape;patients with frequent premature atrial premature beats and premature ventricular contractions, II-III degree atrioventricular block, and short-term atrial tachycardia were irregular;patients with atrial fibrillation scalloped;patients with paroxysmal ventricular tachycardia have a short rod shape.
