摘  要:目的探讨和分析在重症胰腺炎感染后早期肠内营养护理对营养状态的改善效果。方法此次抽选2015年10月—2018年10月在该院医治的重症胰腺炎感染患者(32例)做研究,随机分为乙组(16例)、甲组(16例)。患者均常规治疗和护理,乙组加肠外营养支持,甲组加早期肠内营养护理,总结营养状态、肠功能恢复情况、并发症。结果甲组患者的白蛋白水平(37.61±1.98)g/L、血清总蛋白水平(67.91±3.24)g/L都高于乙组患者[(33.20±2.57)g/L、(62.71±2.58)g/L],差异有统计学意义(t=5.437、5.022,P=0.000)。甲组患者的肠鸣音恢复时间(2.01±0.14)天、肛门排便时间(4.81±0.42)d、肛门排气时间(2.44±0.09)d短于乙组患者[(3.22±0.58)d、(6.13±1.05)d、(3.30±0.38)d],差异有统计学意义(t=8.112、4.669、8.809,P=0.000)。甲组患者并发症的总发生率(0.00%)低于乙组患者(25.00%),差异有统计学意义(χ~2=4.571,P=0.033)。结论在重症胰腺炎感染患者中,早期肠内营养护理可有效改善患者的营养状态,促进肠功能恢复,并降低并发症的发生率。Objective To investigate and analyze the effect of early enteral nutrition nursing on nutritional status after severe pancreatitis infection. Methods This study was conducted in patients with severe pancreatitis(32 cases) who were treated in our hospital from October 2015 to October 2018. They were randomly divided into group B(16 cases)and group A(16 cases). Patients were treated with routine treatment and care. Group B added parenteral nutrition support, group A with early enteral nutrition care, summed up nutritional status, intestinal function recovery, and complications. Results Albumin levels(37.61±1.98) g/L and serum total protein levels(67.91±3.24) g/L were higher in group A than in group B [(33.20±2.57) g/L,(62.71±2.58) g/L], the difference was statistically significant(t=5.437, 5.022,P =0.000). The recovery time of bowel sounds in patients in group A(2.01 ±0.14) days, the time of anal defecation(4.81±0.42) days, and the time of anal exhaust(2.44±0.09)d were shorter than those in group B [(3.22±0.58)d,(6.13±1.05)d,(3.30±0.38)d, the difference was statistically significant(t=8.112, 4.669, 8.809, P=0.000). The overall incidence of complications in group A(0.0%) was lower than that in group B(25.0%), with statistically significant differences(χ^2=4.571, P =0.033). Conclusion In patients with severe pancreatitis infection, early enteral nutrition care can effectively improve the nutritional status of patients,promote intestinal function recovery, and reduce the incidence of complications.
