平板运动试验Tp-Te间期与ST/HR斜率在心肌缺血诊断中的价值

摘　　要：目的探讨平板运动试验Tp-Te间期与ST/HR斜率对心肌缺血的诊断价值。方法将125例行运动平板试验并行冠脉造影的心肌缺血患者,根据运动平板试验和冠脉造影结果分为4组,A组运动平板试验和冠脉造影结果均为阴性者43例,B组运动平板试验阳性、冠脉造影结果阴性者25例,C组运动平板试验阴性、冠脉造影结果阳性者26例,D组运动平板试验和冠脉造影结果均为阳性者31例。比较4组运动峰值时与运动试验前Tp-Te间期,并对4组间Tp-Te差值以及ST/HR斜率进行比较。结果4组运动试验峰值时Tp-Te间期均较运动试验前显著下降(P<0.05)。4组运动试验峰值时与运动试验前Tp-Te间期差值比较差异有统计学意义(P<0.05),其中A组Tp-Te间期差值较另外三组大(P<0.05),而其余三组间比较差异无统计学意义(P>0.05)。4组ST/HR斜率比较差异有统计学意义(P<0.05),其中D组ST/HR斜率较另外三组大(P<0.05),而其余三组间比较差异无统计学意义(P>0.05)。结论在运动平板试验中,应用Tp-Te间期差值结合ST/HR斜率能提髙心肌缺血诊断的准确性。Objective To study the significance of Tp-Te interval and ST/HR slope in the diagnosis of myocardial ischemia in treadmill exercise test.Methods 125 myocardial ischemia patients who underwent treadmill exercise test with coronary angiography were divided into 4 groups according to the results of treadmill exercise test and coronary angiography.Group A had 43 cases with negative results of treadmill exercise test and coronary angiography.Group B had 25 cases with positive treadmill exercise test and negative coronary angiography results.Group C had 26 cases with negative treadmill exercise test and positive coronary angiography results.Group D had 31 cases with positive results of treadmill exercise test and coronary angiography.The Tp-Te intervals of the 4 groups were compared with those before the exercise test,and the Tp-Te difference and the ST/HR slope were compared between the four groups.Results The Tp-Te interval of the 4 groups of exercise tests was lower than that before the exercise test(P<0.05).There was a statistically significant difference between the peak values of the four groups of exercise tests and the Tp-Te interval before exercise test(P<0.05).The difference of Tp-Te interval in group A was larger than that in the other three groups(P<0.05),but the difference among the other three groups was not statistically significant(P>0.05).There was a statistically significant difference in ST/HR slope among the 4 groups(P<0.05).The ST/HR slope of group D was larger than that of the other three groups(P<0.05),but the difference among the other three groups was not statistically significant(P>0.05).Conclusion In the treadmill exercise test,the application of the Tp-Te interval difference combined with ST/HR slope can improve the accuracy of myocardial ischemia diagnosis.
