摘　　要：目的探讨宫腹腔镜联合COOK导丝介入治疗输卵管阻塞性不孕患者的中期疗效。方法将260例输卵管阻塞性不孕患者按随机数字表法分为对照组与观察组,各130例。对照组采取X线透视引导下导丝介入治疗,观察组采取宫腹腔镜联合COOK导丝介入治疗。比较两组手术情况、术后输卵管疏通率、并发症发生率及术后1年妊娠情况。结果两组手术用时及术中出血量比较差异无统计学意义(P>0.05);观察组术后输卵管疏通率显著高于对照组(P<0.05),术后并发症发生率显著低于对照组(P<0.05)。术后1年观察组宫内妊娠率显著高于对照组(P<0.01),未妊娠率显著低于对照组(P<0.01)。结论对输卵管阻塞性不孕患者予以宫腹腔镜联合COOK导丝介入治疗可有效疏通阻塞输卵管,提高宫内妊娠率,减少并发症的发生,中期疗效较好。Objective To explore the mid-term efficacy of hysteroscopy-laparoscopy combined with COOK guide wire intervention in treating patients with tubal obstruction infertility.Methods 260 cases of patients with tubal obstruction infertility were divided into the control group and the observation group according to the random number table method,130 cases in each group.The control group was treated with X-ray guide wire intervention,and the observation group was treated by hysteroscopy-laparoscopy combined with COOK guide wire intervention.The operation condition,patency rate of fallopian tube,incidence of complications and pregnancy after 1 year were compared between the two groups.Results There was insignificant difference in operative time and intraoperative blood loss between the two groups(P>0.05).The patency rate of fallopian tube in the observation group was significantly higher than that in the control group(P<0.05).The postoperative complication rate was significantly lower than that in the control group(P<0.05).The intrauterine pregnancy rate of the observation group was significantly higher than that of the control group at 1 year after operation(PV0.01).and the non-pregnancy rate was sig nificantly lower than that of the control group(P<0.01).Conclusion The combined treatment of hyste roscopy-laparoscopy and COOK guided wire intervention in oviduct obstructive infertility can effectively dredge the fallopian tubes,improve the intrauterine pregnancy rate,reduce the complications,and have a good medium-term effect.
