布比卡因轻与重比重液腰麻在老年人下肢手术中的麻醉效果比较

摘　　要：目的:探讨布比卡因轻比重液、重比重液腰麻在老年人下肢手术麻醉中的应用效果.方法:2017年1月—2018年6月,随机选择收治的老年下肢手术患者75例为对象,随机分为对照组35例和观察组40例.采用布比卡因重比重液腰麻为对照组,布比卡因轻比重液腰麻为观察组,比较两组患者的麻醉优良率和MAP、HR水平以及不良反应发生率.结果:观察组患者的麻醉优良率97.50%与对照组的100.00%比较,差异无统计学意义(P>0.05);观察组患者麻醉后5~10m i n的M A P水平降低、H R水平保持稳定,与对照组M A P、H R水平比较存在显著差异(P<0.05);观察组患者的不良反应发生率5.00%比对照组的34.29%低,差异显著(P<0.05).结论:老年人接受下肢手术时,采用布比卡因轻比重液腰麻的麻醉安全性更高,对患者体内血流动力学影响较小,不良反应低.Objective To investigate the application of bupivacaine in lower extremity surgery anesthesia.Methods From January 2017 to June 2018,75 elderly patients undergoing lower extremity operation were selected by random number table and divided into control group (35 cases) and observation group (40 cases) by single blind method.Bupivacaine heavy specific gravity liguid was used as control group and bupivacaine light specific gravity liguid was used as observation group.The excellent and good rate of anesthesia,MARHR level and adverse reaction rate were compared between the two groups.Results The excellent and good rate of anesthesia in observation group was 97.50% compared with 100.00% in control group.There was no significant difference between observation group and control group (P > 0.05). The MAP level of 5 .lOmin decreased and the level of HR remained stable in the observation group after anesthesia, and there was a difference between the MARHR level of the observation group and the control group (P < 0.05). The incidence of adverse reactions in the observation group was lower than that in the control group (5.00% vs 34.29%, P < 0.05). Conclusion: the anesthetic safety of using bupivacaine light specific gravity liquid for spinal anaesthesia is highei;and the hemodynamic effect is less,so it will not cause the patient to come out.There are more serious adverse reactions.
