苍龟探穴法针刺阿是穴治疗乳腺癌芳香化酶抑制剂所致肌肉骨关节疼痛的临床效果

摘　　要：目的探讨苍龟探穴法针刺阿是穴治疗乳腺癌芳香化酶抑制剂所致肌肉骨关节疼痛的临床效果。方法将广州中医药大学第一附属医院乳腺科2016年7月~2017年9月收治的72例因服用芳香化酶抑制剂导致肌肉骨关节疼痛的乳腺癌患者随机分为对照组(36例)和观察组(36例)。对照组给予口服西药治疗,观察组在对照组的基础上给予苍龟探穴法针刺阿是穴治疗,两组均治疗3个疗程。比较两组患者的临床疗效、视觉模拟评分(VAS)、日常生活活动能力量表(BI)评分及腰椎骨密度(BMD)变化,检测血清细胞因子γ干扰素(IFN-γ)、白细胞介素-4(IL-4)和雌二醇(E2)水平,并观察不良反应发生情况。结果①观察组总有效率明显高于对照组(P<0.05)。②两组患者治疗后VAS评分明显低于治疗前(P<0.01),且观察组VAS评分明显低于对照组(P<0.01)。③两组患者治疗后BI评分及BMD值均明显高于治疗前(P<0.05),且观察组BI评分及BMD值均明显高于对照组(P<0.05)。④与治疗前比较,两组患者治疗后IFN-γ水平显著降低,IL-4水平显著升高(P<0.01),E2水平无明显变化(P>0.05);与对照组比较,观察组IFN-γ水平显著降低,IL-4水平显著升高(P<0.01),但两组患者血清E2水平比较差异无统计学意义(P>0.05)。⑤两组患者在治疗过程中均未发生不良反应。结论苍龟探穴法针刺阿是穴可有效改善乳腺癌患者因使用芳香化酶抑制剂引起的肌肉骨关节疼痛,并可显著提高患者生活能力,且安全可行,值得临床推广使用。Objective To explore the clinical effect of Canggui Tanxue acupuncture at ashi point in the treatment of muscle, bone and joint pain induced by aromatase inhibitor of breast cancer. Methods Seventy-two breast cancer patients with muscle, bone and joint pain because of taking aromatase inhibitor admitted to Galactophore Department of the First Affiliated Hospital of Guangzhou University of Chinese Medicine from July 2016 to September 2017 were randomly divided into control group (36 cases) and observation group (36 cases). The control group was treated with oral Western medicine, based on the control group, the observation group was given Canggui Tanxue acupuncture at ashi point. Both groups were treated for 3 courses. The clinical effect, the scores of visual analogue scale (VAS), Barthel index (BI) and the changes of lumbar spine bone mineral density (BMD) were compared between two groups. The levels of serum cytokine interferon-γ(IFN-γ), interleukin-4 (IL-4), estradiol (E2) were detected and the occurrence of adverse reactions was observed. Results ①The total effective rate of the observation group was significantly higher than the control group (P<0.05).②After treatment, the VAS scores of the two groups were significantly lower than those before treatment (P<0.01), and the observation group was significantly lower than the control group (P<0.01).③After treatment, the BI score and BMD values of lumbar spine in the two groups were significantly higher than those before treatment (P<0.05), which of the observation group were significantly higher than the control group (P<0.05).④Compared with before treatment, the levels of IFN-γ were decreased significantly, the levels of IL-4 were increased significantly in the two groups after treatment (P<0.01), while the levels of E2 did not change significantly (P >0.05);compared with the control group, the levels of IFN-γ were significantly decreased and the levels of IL-4 were significantly increased in the observation group (P<0.01), but there was no significa
