清热利湿通淋汤联合五水头孢唑林钠治疗急性泌尿系感染及其对炎性应激状态的影响

摘　　要：目的:研究清热利湿通淋汤联合五水头孢唑林钠治疗急性泌尿系感染的疗效及其对患者血清炎性因子的影响。方法:选择2016—08～2018—06在我院接受治疗的急性泌尿系感染患者128例,根据治疗方法分为对照组(n=64)和观察组(n=64)。对照组采用五水头孢唑林钠静脉滴注治疗,观察组在对照组治疗基础上加用清热利湿通淋汤治疗。连续治疗5 d后比较两组的临床疗效、细菌清除率、症状改善时间、治疗前后血清白介素-6(interleukin-6,IL-6)、白介素-8(interleukin-8,IL-8)、肿瘤坏死因子(tumor necrosis factor-α,TNF-α)和组织因子(tissue factor,TF)等炎症因子水平及不良反应发生情况。结果:观察组的治疗总有效率为93.75%(60/64),高于对照组的79.69%(51/64),差异具有统计学意义(P<0.05)。观察组治疗后细菌清除率为100.00%,显著高于对照组的75.00%(P<0.05)。观察组体温恢复至正常时间和尿路刺激征消失时间均显著少于对照组,差异具有统计学意义(P<0.05)。两组治疗后血清IL-6、IL-8、TNF-α和TF水平均较治疗前显著降低,且观察组治疗后血清IL-6、IL-8、TNF-α和TF水平显著低于对照组(P<0.05)。两组治疗不良反应发生率比较差异无统计学意义(P>0.05)。结论:清热利湿通淋汤联合五水头孢唑林钠治疗急性泌尿系感染可有效提高细菌清除率,改善患者炎症状态,且安全性较高。Objective:To study the effect of Qingre Lishi Tonglin decoction combined with cefazolin sodium pentahydrate on acute urinary tract infection and its influence on serum inflammatory factors.Methods:A total of 128 patients with acute urinary tract infection treated in our hospital from August 2016 to June 2018 were enrolled.According to the treatment methods,they were divided into control group(n=64) and observation group(n=64).The control group was treated with cefazolin sodium pentahydrate intravenous drip,while the observation group was treated with Qingre Lishi Tonglin decoction on the basis of the control group.Two groups were treated for 5 days continuously,the clinical efficacy,bacterial clearance rate and symptom improvement time,level of interleukin-6(IL-6),interleukin-8(IL-8),tumor necrosis factor-α(TNF-α) and tissue factor(TF) were detected before and after treatment,and the incidence of adverse reactions was compared between the two groups.Results:The total effective rates of the observation group was 93.75%(60/64),which was higher than that of the control group 79.69%(51/64), respectively the difference was statistically significant(P<0.05).The bacterial clearance rate in the observation group after treatment was 100.00%,which was significantly higher than that in the control group 75.00%(P<0.05).The recovery time of body temperature and the disappearance time of urinary tract irritation syndrome in the observation group were significantly shorter than those in the control group(P<0.05).The serum levels of IL-6,IL-8,TNF-α and TF after treatment in the two groups were significantly lower than those before treatment.The serum levels of IL-6,IL-8,TNF-α and TF in the observation group were significantly lower than those in the control group(P<0.05) after treatment.There was no significant difference in incidence of adverse reactions between the two groups(P>0.05).Conclusion:Qingre Lishi Tonglin decoction combined with cefazolin sodium pentahydrate can effectively improve bacterial clearance and inflamm
