摘 要:目的探讨分层级管理模式在血透室护理管理中的临床价值。方法我院于2018年1月运用分层级管理模式,选择2017年3月至2017年12月45例血透室患者为参照组,2018年1月至2018年10月45例血透室患者为试验组,比较两组的自我护理能力、并发症发生情况,分析管理前、后护士的护理工作质量、患者对护理满意度以及护士对工作满意度。结果试验组自我护理能力总评分、自护责任感、健康知识水平、自我概念、自我护理技能评分均显著高于参照组,差异具有统计学意义(P<0.05);管理后,护士的护理宣教、专科护理技能、应急处理、护理安全、护理文书、基础护理、整体护理评分均显著高于管理前,差异具有统计学意义(P<0.05);试验组高血钾、水负荷过高、心衰、血压异常、内瘘闭塞发生率均显著低于参照组,差异具有统计学意义(P<0.05);管理后,患者对护理满意度和护士对工作满意度评分均显著高于管理前,差异具有统计学意义(P<0.05)。结论在医院血透室护理管理中,分层级管理模式应用效果突出。Objective To explore the clinical value of hierarchical management mode in nursing management of hemodialysis room.Methods The hierarchical management model was used in our hospital in January 2018.A total of 45 patients in hemodialysis room from March 2017 to December 2017 were selected as reference group, and 45 patients in hemodialysis room from January 2018 to October 2018 were selected as experimental group.The self-care ability and complications of the two groups were compared.The quality of nursing work, patients'satisfaction with nursing and nurses' satisfaction with work before and after management were analyzed.Results The total score of self-care ability, scores of self-care responsibility, health knowledge level, self-concept and self-care skill of the experimental group were significantly higher than those of the reference group, with statistically significant differences (P<0.05).After management, the scores of nursing education, specialized nursing skills, emergency treatment, nursing safety, nursing documents, basic nursing and overall nursing of nurses were significantly higher than those before management, and the differences were statistically significant (P<0.05).The incidences of high blood potassium, excessive water load, heart failure, abnormal blood pressure and internal fistula occlusion in the experimental group were significantly lower than those in the reference group, and the differences were statistically significant (P<0.05).After management, patients' satisfaction with nursing and nurses' satisfaction with work were significantly higher than those before management, and the differences were statistically significant (P<0.05).Conclusion In the nursing management of hospital hemodialysis room, the hierarchical management model has a prominent effect.
