基于互联网远程量化连续监测尿LH预测排卵提高受孕率的研究

摘  要:目的探讨基于互联网远程量化连续监测尿促黄体生成素(LH)预测排卵对受孕率的影响。方法选取于我院行孕前检查的286位女性,随机平均分为两组。实验组采用优迈斯科仪进行尿LH检测,当测到LH峰值时指导夫妇同房;对照组不作干预。对比实验组尿液LH峰值检出情况与B超验证排卵的符合率,及两组的妊娠与母婴情况。结果实验组尿液LH峰值检出情况与B超验证排卵的符合率为93.01%,敏感度为97.83%,特异度为84.31%,漏诊率为2.17%,误诊率为15.69%,阳性预测值为91.84%,阴性预测值为95.56%,阳性似然比为6.24,阴性似然比为0.26, LH峰值与B超结果具有中度一致性(χ~2=3.842,P=0.001)。两组的自然流产率、并发症发生率、剖宫产率、早产率、新生儿出生缺陷率比较均无统计学差异(P>0.05)。实验组的受孕率、终止妊娠孕周、新生儿体重、 Apgar评分均高于对照组(P <0.05)。结论互联网远程量化连续监测尿LH可有效预测排卵,提高受孕率。Objective To explore the influence of ovulation prediction based on internet remote quantization continuous monitoring of urinary luteinizing hormone(LH) on conception rate. Methods 286 women with pre-pregnancy examination were selected and randomly divided into two groups equally. The experimental group used Youmaisike Tester to detect the urinary LH, and the couples were instructed to have intercourse when the urinary LH peak value was detected. The control group was not given intervention. The coincidence rate of urinary LH peak value with B-ultrasound ovulation verification in experimental group, and the pregnancy and maternal and infant conditions of two groups were compared. Results In experimental group, the coincidence rate of urinary LH peak value with B-ultrasound ovulation verification was 93.01%, and the sensitivity, specificity, missed diagnosis rate, misdiagnosis rate, positive predictive value, negative predictive value, positive likelihood ratio and negative likelihood ratio were respectively 97.83%, 84.31%, 2.17%, 15.69%, 91.84%,95.56%, 6.24 and 0.26. LH peak value and B-ultrasound results had a moderate agreement(χ~2= 3.842, P=0.001). No statistical difference was found in the spontaneous abortion rate, incidence of complications, cesarean section rate, preterm delivery rate and neonatal birth defect rate between the two groups(P >0.05). The pregnancy rate, terminated gestational weeks, neonatal weight and Apgar score of the experimental group were higher than those of the control group(P<0.05). Conclusions Internet remote quantization continuous monitoring of urinary LH can effectively predict the ovulation and improve the pregnancy rate.
