摘  要:目的探讨粪产碱杆菌所致医院感染的临床易感因素、对常用抗菌药物的耐药性及其防治措施,为临床合理使用抗菌药物及预防医院感染提供依据。方法对2015年1月-2017年12月临床分离的32株粪产碱杆菌所致医院感染进行回顾性分析。结果共分离培养到32株粪产碱杆菌。粪产碱杆菌所致医院感染与患者的年龄、伴有慢性基础疾病、接收呼吸机治疗、长期使用大量广谱抗菌药物密切相关;药敏结果显示,粪产碱杆菌对替加环素、多黏菌素、美罗培南、亚胺培南、哌拉西林/他唑巴坦、头孢哌酮/舒巴坦、头孢替坦、丁胺卡那霉素、磺胺甲恶唑/甲氧苄啶、氨苄西林/舒巴坦、哌拉西林、妥布霉素、左氧氟沙星等敏感性较好,耐药率<20.00%。结论应加强粪产碱杆菌的耐药性监测,规范抗菌药物的临床应用,预防和控制粪产碱杆菌引起的医院感染。Objective To investigate the clinical distribution of Alcaligenes faecalis and the drug resistance to commonly used antibiotics, so as to provide guidance for reasonable clinical use of antibiotics and for the prevention of nosocomial infection. Methods A retrospective analysis of 32 hospital infections caused by Alcaligenes faecalis clinically isolated from January 2015 to December 2017 was performed. Results A total of 32 strains of A.faecalis had been isolated from sputum specimens. The nosocomial infection induced by A.faecalis was closely associated with the age, complication of underlying diseases, mechanical ventilation, and use of high dose broad spectrum antibiotics. The results of the drug susceptibility testing showed that the drug resistance rates of A.faecalis to tigecycline, polymyxin, meropenem, imipenem, piperacillin/tazobactam, cefoperazone/sulbactam, cefotetan, amikacin, trimethoprim/sulfamethoxazole, ampicillin/sulbatan, piperacillin, tobramycin and levofloxacin were less than 20.00%. Conclusion It is necessary to strengthen the surveillance of drug resistance of A.faecalis and standardize the clinical use of antibiotics, so as to prevent and control the nosocomial infection caused by A.faecalis.
