摘  要:目的:观察纤维支气管镜介入治疗儿童难治性肺炎的应用效果及安全性。方法:将2016年1月~2018年12月某院儿科收治的114例难治性肺炎患儿纳入研究对象,将其随机分为支气管镜组和对照组各57例,对照组患儿给予抗感染、吸氧、雾化、止咳、化痰、吸痰等治疗,支气管镜组患儿在此基础上,给予纤维支气管镜介入治疗。于治疗1周后进行临床疗效评价,并记录两组患儿外周血WBC和CRP异常持续时间、发热持续时间、住院时间及纤维支气管镜介入治疗的不良反应。结果:支气管镜组患儿外周血WBC异常时间、CRP异常时间、热程及住院时间均明显短于对照组(P<0.05);支气管镜组显效率和总有效率明显高于对照组(P<0.05),无效率明显低于对照组(P<0.05),有效率与对照组相比无统计学差异(P>0.05);接受纤维支气管镜介入治疗的57例患儿中,未出现严重并发症。结论:对难治性肺炎患儿进行纤维支气管镜介入治疗,可有效缓解患儿的疾病症状,缩短发热及住院时间,提高临床治疗效果,且无严重并发症,有利于患儿早日康复。Objective: To observe the effect and safety of fiberoptic bronchoscope interventional therapy in the treatment of pediatric refractory pneumonia. Methods: 114 children with refractory pneumonia admitted to the department of pediatrics of a hospital from January 2016 to December 2018 were included in the study, and were randomly divided into bronchoscope group and control group, 57 cases each. The control group was treated with anti-infection, oxygen inhalation, atomization, cough relief, phlegm resolving, sputum suction, correction of water and electrolyte balance disorders. On the basis of the treatment in the control group, the children in the bronchoscope group were given interventional treatment with fiberoptic bronchoscope. The clinical efficacy was evaluated after 1 week of treatment. The abnormal duration of WBC and CRP in peripheral blood, duration of fever, length of hospitalization and adverse reactions of fiberoptic bronchoscopy interventional therapy were recorded. Results: The duration of abnormal WBC, CRP, fever and hospitalization in bronchoscopy group were significantly shorter than those in control group ( P <0.05). The marked efficiency and total effective rate of bronchoscopy group were significantly higher than those of control group ( P <0.05), and the inefficiency was significantly lower than that of control group ( P <0.05). There was no significant difference in the effective rate between bronchoscopy group and control group ( P >0.05). There were no serious complications in 57 children who received interventional treatment with fiberoptic bronchoscopy. Conclusion: The fiberoptic bronchoscope interventional therapy in the treatment of pediatric refractory pneumonia can effectively relieve the symptoms of the disease, shorten the fever and hospitalization time, improve the clinical treatment effect, without serious complications, and is conducive to the early recovery of the children.
