摘 要:目的观察心理干预应用于肿瘤门诊导诊工作中的效果。方法将北京大学肿瘤医院2016年1月至2017年1月间肿瘤门诊应用心理干预前导诊接待的158例患者纳入对照组,将2017年2月至2018年2月间肿瘤门诊应用心理干预后导诊接待的158例患者纳入观察组。对比两组的护理质量指标:挂号等候时间、辅助检查等候时间、沟通技巧、健康知识、患者就诊表述;统计导诊纠纷发生率;评估患者就诊前心理状态;调查2组患者对导诊工作的整体满意程度。结果导诊纠纷发生率观察组为8.23%,对照组为17.72%,观察组导诊纠纷发生率与挂号等候时间、辅助检查等候时间均少于对照组(P<0.05);观察组沟通技巧、肿瘤知识、就诊表述评分均高于对照组(P<0.05);观察组心理干预后SAS、SDS、HAMA及HAMD评分均低于对照组(P<0.05);观察组患者总满意率为87.34%,对照组患者总满意率为72.78%,观察组患者满意程度与总满意率均优于对照组(P<0.05)。结论心理干预能够改善患者的心理状态,优化患者就诊时的表述水平、有效提升导诊服务质量、降低导诊纠纷发生率,提高患者对导诊工作的整体满意程度。Objective To observe the effect of psychological intervention on the guidance for outpatients with cancer.Methods From January 2016 to January 2017,158 patients who were admitted to the cancer outpatient clinic before psychological intervention were included in the control group.From February 2017 to February 2018,158 patients receiving psychological intervention in the Outpatient Department of cancer were included in the observation group.The indicators of nursing quality between the two groups including waiting time for registration,auxiliary examination waiting time,communication skills,health knowledge,experience of visiting and the incidence rate of disputes were compared.The psychological state of patients before treatment was evaluated.And the overall satisfaction degree of patients in the two groups with the guided diagnosis was investigated.Results The incidence of disputes was 8.23%in the observation group which was significantly less than that(17.72%)in the control group(P<0.05).The communication skills,tumor knowledge and presentation scores of patients in the observation group were all higher than those in the control group(P<0.05).After psychological intervention,the scores of SAS,SDS,HAMA and HAMD in the observation group were lower than those in the control group(P<0.05).The total satisfaction rate of patients in the observation group was 87.34%which was significantly higher than that(72.78%)in the control group(P<0.05).Conclusions Psychological intervention can improve the psychological state of patients before treatment and reduce the incidence of disputes,which improves the overall satisfaction of patients receiving guided diagnosis.
