活血洗剂熏洗治疗Ⅰ、Ⅱ期肛裂临床观察

摘　　要：目的观察活血洗剂熏洗对Ⅰ、Ⅱ期肛裂患者的临床疗效。方法将86例肛裂患者按照随机数字表法随机分为两组,每组43例,对照组应用高锰酸钾洗剂治疗,治疗组应用活血洗剂熏洗治疗,观察两组患者在治疗第1天、第3天、第7天时创面分泌物、创面疼痛、创面肉芽生长情况,以及血清肿瘤坏死因子-α(tumor necrosis factor-alpha, TNF-α)、白细胞介素-6(interleukin-6, IL-6)水平,随访时观察两组患者肛管静息压(anal resting pressure,ARP)、肛管最大收缩压(anal maximum contraction pressure,AMCP)及大便失禁严重度评分(Wexner评分)。结果活血洗剂可降低患者的创面分泌物积分、创面疼痛积分、创面肉芽生长积分、ARP、Wexner积分(P<0.05),降低血清TNF-α、IL-6水平(P<0.05),且以上指标降低程度明显大于对照组(P<0.05);活血洗剂对AMCP无明显影响(P>0.05)。结论活血洗剂可减少肛裂患者的创面分泌物,减轻创面疼痛,促进创面肉芽生长,降低炎症反应,改善肛门功能。Objective To investigate the clinical effect of fumigation-washing therapy with Huoxue Lotion in the treatment of stage Ⅰ/Ⅱ anal fissure. Methods A total of 86 patients with anal fissure were divided into treatment group and control group using a random number table,with 43 cases in each group. The patients in the control group were treated with potassium permanganate lotion, and those in the treatment group were given fumigation-washing therapy with Huoxue Lotion. The two groups were observed in terms of wound secretion, wound pain, wound granulation growth, and serum levels of tumor necrosis factor-α(TNF-α) and interleukin-6 (IL-6) on days 1, 3, and 7 of treatment. The patients were followed up to observe anal resting pressure (ARP), anal maximum contraction pressure (AMCP), and Wexner score for the severity of fecal incontinence. Results Huoxue Lotion significantly reduced wound secretion score, wound pain score, wound granulation growth score, ARP, Wexner score, and serum levels of TNF-α and IL-6 (P< 0.05), and the treatment group had significantly greater reductions in these indices than the control group (P< 0.05). Huoxue Lotion had no significant effect on AMCP (P>0.05). Conclusion Huoxue Lotion can reduce wound secretion, alleviate wound pain, promote wound granulation growth, reduce inflammatory response, and improve anal function.
