摘 要:目的比较新型便携式无线超声与传统有线超声在经皮肾镜碎石取石术(PCNL)术中引导经皮肾穿刺的安全性与有效性。方法回顾分析2017年11月至2019年1月惠州市中心人民医院收治行PCNL的单侧肾结石伴中度或以上肾积水患者的临床资料,共84例,其中39例(实验组)由新型便携式无线超声引导穿刺,45例(对照组)由传统有线超声机引导穿刺,两组手术均由同一组泌尿外科医师完成。结果两组患者的术前临床资料差异无明显统计学意义(P>0.05)。所有患者手术均取得成功,无一例中转开放手术,无胸腔及肠道损伤等并发症发生;两组患者的穿刺引导时间、穿刺深度,手术时间、出血量、清石率、术后住院天数差异均无统计学意义(P>0.05)。结论新型便携式无线超声在肾结石伴中度或以上肾积水的PCNL术中引导穿刺是安全有效的,其能达到传统有线超声相近的效果,术中使用方便,具有一定的临床应用价值。Objective To compare the safety and effectiveness of the new portable wireless ultrasound and traditional wired ultrasound in puncture guiding of percutaneous nephrolithotomy (PCNL). Methods Retrospective analysis was performed on 84 cases of unilateral renal calculus with hydronephrosis treated with PCNL in Huizhou Central Hospital from Nov 2017 to Jan 2019. Thirty nine patients (experimental group) were guided by the new portable wireless ultrasound, and 45 patients (control group) were guided by a traditional wired ultrasound. Both groups of surgery were performed by the same group of urologists. Results There were no significant statistical differences in base data between the two groups (all P>0.05). All the operations were completed successfully. And no patient was converted to open surgery, without complications such as thoracic or intestinal damage. There were no significant statistical differences in puncture guidance time, puncture depth, operation time, bleeding volume, calculus clearing rate, postoperative hospitalization days between the two groups (all P>0.05). Conclusion The new portable wireless ultrasound is safe and effective for renal calculus with hydronephrosis in PCNL, and it can achieve the similar effect of the traditional wired ultrasound, which is convenient to use and has certain clinical value.
