摘  要:作为多种肿瘤的一线治疗策略,放化疗虽可有效抑制肿瘤生长,但副作用明显,骨髓抑制是其最严重的副作用。本研究通过调研相关临床及实验研究,总结了针灸治疗放化疗后骨髓抑制的选穴规律、有效的治疗方式及起效机制。结果发现多种针灸治疗方式均可一定程度改善放化疗后骨髓抑制,艾灸为最常用的临床治疗手段,足三里是最常用选穴。针灸修复造血微环境、调节Notch信号通路促进造血细胞增殖可能是针灸治疗放化疗后骨髓抑制的起效机制之一。因此,针灸疗法可有效改善放化疗后骨髓抑制的部分起效机制已被揭示,但仍有待于深入系统研究。As one of the first-line therapies for a variety of tumors, radiation and chemotherapy can effectively inhibit tumor growth, but the side effects are obvious. Myelosuppression is the most serious side effect. Through the investigation of related clinical and experimental studies, this study summarized the rules of the acupoint selection, the effective treatments as well as the underlying mechanisms and pathways of acupuncture and moxibustion treating myelosuppression induced by chemoradiotherapy. It was found that several methods of acupuncture and moxibustion could relieve myelosuppression. Moxibustion is the most commonly applied and the use of Zusanli(ST36) is most frequent in clinical trials. The potential mechanisms of acupuncture and moxibustion acting on myelosuppression involve promoting the proliferation of hematopoietic cells via improving the bone marrow micro-niche and Notch signaling pathway. In conclusion, acupuncture and moxibustion could improve chemoradiotherapy-induced myelosuppression, and some related mechanisms have been disclosed, but need further studied.
