药剂师消化系统用药调剂错误的原因及处理措施

摘　　要：研究了2016年12月-2018年12月我院开出的4 722份消化系统用药处方,统计其调剂错误情况,分析错误原因,并提出改进措施。其中,调剂错误情况42份,发生率为0.89%。分析发现,医师处方书写不规范、药剂师责任心不强、缺乏专业临床知识、药品摆放不符合规定是引起药剂师用药调剂错误的主要原因。4722 prescriptions for digestive system drugs issued in our hospital from December 2016 to December 2018 were studied.The errors in dispensing and the reasons for the errors were analyzed.The improvement measures were put forward.Among 4722 digestive system medications,42 were misadjusted,with an incidence of 0.89%.It was found that the main reasons for pharmacists’errors in drug dispensing were the improper writing of prescriptions,the weak sense of responsibility of pharmacists,the lack of professional clinical knowledge and the inconsistent dispensing of drugs.Drug dispensing errors in digestive system are very common.
